Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2030985 Primary Facility 2012-01-03 Routine/Complete Inspection No Violations Noted Site had been constructed in accordance with the E&S plan located onsite. These erosion controls should effectively minimize accelerated erosion or sedimentation provided they are maintained properly. Drill rig located onsite however drilling had not commenced. Drill pit had been constructed but not lined. Site construction had been completed on 12/30/11. Due to the amount of water accumulated in the drill pit and the soils information included in the E&S plan the Department recommends that the operator dispose of the drill cuttings in accordance with Chapter 78.63 and land farm the drill cuttings as opposed to encapsulation in accordance with Chapter 78.62. Should the operator desire to dispose of the drill cuttings via encapsulation in accordance with 78.62, the Department requests that the operator forward documentation no later than January 24, 2012 which documents that any seasonal high groundwater table does not rise to within 20 inches of the bottom of the excavated drill pit located onsite.
2032342 Primary Facility 2012-01-04 Drilling/Alteration No Violations Noted US Energy Rig#5 RD20 drilling at 240' at time of inspection.
2033628 Primary Facility 2012-01-12 Routine/Complete Inspection No Violations Noted Drilling ongoing. No E&S issues. Recommended that the operator temporarily stabilize areas that would not be disturbed further and requested documentation should the operator desire to landfarm the drill cuttings.
2069272 Primary Facility 2012-05-17 Routine/Complete Inspection No Violations Noted At the time of inspection the well was drilled and equipped for production. The well apparatus and brine tank onsite appear to be sound condition. The site is seeded and mulched but doesn’t meet the restoration criteria of 70% vegetative coverage. The Department suggest the Operator continue to monitor the site to ensure this criteria is met.
