2146059 Primary Facility 2013-03-07 Routine/Complete Inspection No Violations Noted Routine inspection. No issues noted.
2170259 Primary Facility 2013-05-30 Routine/Complete Inspection No Violations Noted At the time of inspection the well appeared to be drilled and equipped for production. The well apparatus and brine tanks appear to be in sound working condition. The Operator appears to be in the beginning phases of site restoration, the pit contents and liner have been removed and the backfilling of the pit appears to have begun. No accelerated erosion or sedimentation observed in relation to the well pad or access road. The Department suggests the operator continue to monitor the site weekly and after significant precipitation events to ensure the site stability is maintained.
2354924 Primary Facility 2015-03-24 Site Restoration No Violations Noted Inspection in repose to the Operators submission of the "Well Site Restoration Report" on 2/11/2015. At the time of inspection all earth disturbance activities are complete and PCSM BMPs have been installed. The Department notes that the various temporary BMPs are in place and with vegetative coverage, however the site doesn’t not appear to meet the 70% uniform vegetative coverage criteria needed for restoration. The site will we be reevaluated in May/June 2015 to verify 70% uniform vegetative growth and removal of temporary BMPs.
2556897 Primary Facility 2017-01-30 Routine/Partial Inspection No Violations Noted At the time of inspection, the well appeared to be drilled and equipped for production. Well apparatus and brine tanks appeared to be in sound condition. No accelerated erosion or sedimentation observed in relation to the well pad or access road. The Department suggests the operator continue to monitor the site weekly and after significant precipitation events to ensure the site stability is maintained.
2834675 Primary Facility 2019-01-28 Routine/Partial Inspection In Compliance with Policy RTPT inspection due to snow cover.

At time of inspection well appears equipped for production. Two brine tanks on site. Site and access road snow-covered. Department is unable to determine if any E and S issues exist at this time.
