The well apparatus and brine tanks located on site appear to be sound condition. The erosion and sedimentation plan was located onsite and appears to be implemented as designed. The Associate BMPS appear to be properly installed and maintained. No accelerated erosion and sedimentation observed in relation to the access road or well pad area at the time of inspection. The pit located onsite appears to be in sound condition and had the required 2' of freeboard. The Department suggests the operator continue to monitor the site weekly and after significant precipitation event to ensure the stability of the site in maintained.

According to the well record submitted to the Department by the Operator the well was drilled to total depth on 5/14/2012 making the restoration date 2/14/2013, the subsequent violation is for the failure to restore the site within 9 months or file for a site restoration extension within the same time frame.
