2165097 Primary Facility 2013-05-09 Routine/Complete Inspection No Violations Noted At the time of inspection the Operator indicated the well reached total depth on 5/9/2013. The Department reminds the Operator the restoration date will be 9 months from this date, which is 2/9/2014. The drill rig was onsite during the inspection and places on secondary containment. The Erosion and Sedimentation Plan was on site. No accelerated erosion or sedimentation observed in relation to the access road or well pad area. The perimeter E&S BMPs appear to properly installed and functioning properly. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site in maintained.
2167543 Primary Facility 2013-05-14 Drilling/Alteration No Violations Noted 5/14/2013 Rig moving off of pad.
2183354 Primary Facility 2013-07-10 Complaint Inspection No Violations Noted Inspection the result of a complaint filed with the Department on 7/10/2013. The complaint consist of excessive water run off presumed from the well pad and access road adjacent to the land owners property. The landowner also complained of excessive erosion and sedimentation which filled in and series of fish ponds. During the inspection the Department observed what appeared to be a make shift dam made of tires above the fish ponds approximately 75 yards. During the recent significant precipitation event the make shift dam appeared to fail and fill in the fish ponds. The Department also check the perimeter BMPs of the well site. The Department found several small areas that need attention, however they do not appear to be contributing accelerated erosion or sedimentation to the drainage feature holding the fish ponds.
2195665 Primary Facility 2013-08-14 Routine/Complete Inspection No Violations Noted At the time of inspection the Operator was actively stimulating the well. The equipment used for stimulation was placed on containment and appeared to in sound condition.
