2252360 Primary Facility 2014-03-10 Routine/Complete Inspection No Violations Noted At the time of inspection the Operator was on site preparing the drill rig to begin drilling on 3/11/2014. A spud was received by the Department on 3/10/2014. During the inspection the E&S plan was on site and appeared to be installed and maintained. The drill pit located on site appeared to be sound condition. Also observed was a stream crossing with approximately 30' of fill located on top of a plastic 24" pipe. On 3/11/2014, Biologist Supervisor Dan Counahan will be in the area to inspect the crossing.
2255058 Primary Facility 2014-03-12 Follow-up Inspection No Violations Noted Follow up inspection with Biologist 3 Dan Counahan in regards to the stream crossing located on the access road leading to the well site. After further review, the Department is aware the drainage area above the crossing is less than 100 acres, however the material used to construct the crossing appears to be unstable at best and poses a potential threat to waters of the commonwealth. The Department suggests the Operator submit a Chapter 105 permit (Standard Joint Permit Application) for this permanent road crossing from the Oil and Gas Office. If the proposed limit of disturbance is more than 5,000 square feet, but less than 5 acres, the E&S Plan should also be included with the 105 Permit for Department review and approval. If the LOD is over 5 acres, the applicant should submit an ESCGP-2 to the Department for a separate review and approval.
2259109 Primary Facility 2014-03-31 Drilling/Alteration No Violations Noted Waiting on frac. Well shut in on 4 ½” x 2” swage. 7” x 4 ½” annulus closed. Pit lined and freeboard maintained.
2380330 Primary Facility 2015-06-18 Routine/Complete Inspection No Violations Noted At the time of inspection the well was drilled and equipped for production. The brine tank, pump jack and well apparatus appear to be in sound condition. The site appeared to be restored. No observation of accelerated erosion or sedimentation in relation to the well site or access road.
2649450 Primary Facility 2017-10-19 Routine/Complete Inspection No Violations Noted At the time of inspection well equipped for production. Two brine tanks within earthen dike and appears to be structurally sound. No E&S issues.
