The inspection at the request of the Operator to inspect perimeter BMP controls prior earth disturbance. At the time of inspection the Erosion and Sedimentation plan was on site and implemented as designed. No observation of accelerated E&S issue in relation to proposed well pad area or the access road. The Department suggests that the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site in maintained.

2163808 Primary Facility 2013-05-06 Routine/Complete Inspection No Violations Noted At the time of inspection the Operator was in the process of constructing the site.
2174955 Primary Facility 2013-06-13 Routine/Complete Inspection In Compliance with Policy Inspection the result of the Operator reporting a breached BMP during a precipitation event. The Operator is in compliance with the Departments notification policy for a BMP breach. During the inspection the Department notes that an infiltration ditch located closest to the access road failed during a heavy precipitation event in the early morning hours on 6/13/2013. The failure resulted in material from the infiltration ditch being washed into a 32" silt sox, which did not contain the material and also failed. The two failures resulted is accelerated erosion of the access road and scouring of a hillside below the excavated pad area. The precipitation event appears to exceed the Departments criteria for construction standards. The Operators response was to rebuild the failed portion of the infiltration ditch, add an additional rock outlet and repair the silt sox. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2175885 Primary Facility 2013-06-13 Drilling/Alteration No Violations Noted Conductor being cemented in at time of inspection. Inspection conducted w/OGI J. Branan.
2177613 Primary Facility 2013-05-24 Routine/Complete Inspection No Violations Noted At the time of inspection the drill rig was located onsite and placed on containment. The Department notes that the Operator replaced the silt sox, installed an additional rock outlet, and repaired the failed earthen dike surrounding the well site. The Department has also notes that the additions have been added to the E&S plan. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.

2209076 Primary Facility 2013-10-02 Routine/Complete Inspection No Violations Noted At the time of inspection the well was drilled to KOP. The E&S plan was onsite and all BMPs appeared be installed and maintained as outlined in the plan. No observation of accelerated erosion or sedimentation in relation to the well pad or access road. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2216394 Primary Facility 2013-10-25 Routine/Complete Inspection No Violations Noted At the time of inspection the drill rig was on site and placed on containment. The Operator indicated that drilling had not started. The E&S plan was onsite and all BMPs appeared be installed and maintained as outlined in the plan. However, the Department suggested to the Operator during the inspection that routine maintenance be performed to the silt sock on either side of the road, located directly behind the onsite camps and company trailers. The Department observed some small areas where water was either traveling along the silt sock or under the silt sock. No observation of accelerated erosion in those areas, however preventative maintenance was suggested. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
