2156469 Primary Facility 2013-04-12 Incident- Response to Accident or Event No Violations Noted Inspection the result of a spill reported to the Department on 4/12/2013. The Operator indicated that 2 barrels of drill cuttings were spilled onto containment between the shaker and the drill pit. The Department notes that this spill is in compliance with policy. The Operator was in the process of vacuuming the spilled drill cuttings out of the containment during the inspection. The Erosion and Sedimentation Plan was on site. No accelerated erosion or sedimentation observed in relation to the access road or well pad area. The perimeter E&S BMPs appear to properly installed and functioning properly. Various pits located on site appear to be in sound condition and have the required 2' of freeboard. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site in maintained.
2188802 Primary Facility 2013-08-02 Routine/Complete Inspection Pending At the time of inspection the well has been drilled and stimulated. During the inspection the Operator was in the process of reconfiguring the well site to start flow back. During the inspection the Department observed various holes and tears in the drill pit located adjacent to the well heads. The tears are located in pit labeled #1. The observed holes and tears are located in southeast corner of pit #1. The Department suggest the Operator remove the liquid and solid fractions contained in the pit. Upon removal of pit contents contact the Department to coordinate an inspection of the pit liner. Also during the inspection the Department notes that Operator appears to be using a solidifier and at this time the Department does not have an OG 71 for the addition of solidifiers to drill pit. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site in maintained.
2209112 Primary Facility 2013-10-01 Routine/Complete Inspection Violation(s) Noted At the time of inspection the well has been drilled and stimulated. The Operator was in the process of equipping the wells for production. The Department notes that the Drill Pit located onsite has been backfilled. The final report addressing the torn pit liner and the subsequent release of pollutional material has been reviewed. The following violations are issued for the Operators failure to detect and address a compromised pit liner, which resulted in a spill to ground and the addition of portland cement to drill cuttings without prior approval.
