2349127 Primary Facility 2015-03-02 Incident- Response to Accident or Event In Compliance with Policy Inspection in response to the operator reporting a 5 gallon hydraulic oil spill. The spill occurred on 2/27/2015 @ 17:00. At the time of inspection the spill appeared to be remediated. The Operator indicated that the spill occurred when a loader ruptured a hydraulic line resulting in a spill to the ground. At the Operator immediately reported the spill and began remediation. MSDS sheets provided.
2349128 Primary Facility 2015-03-02 Incident- Response to Accident or Event Violation(s) Noted Inspection in response to the operator reporting a 25 barrel drilling mud spill. The spill occurred on 2/28/2015 @ 01:20. At the time of inspection the spill appeared to be remediated. The Operator indicated that the spill occurred when an open tank located onsite was overfilled. The Operator indicated that 20 Barrels of salt water based drilling mud was lost to containment and the remaining 5 barrels was lost to the ground. The Department requests the disposal manifest associated with the removal of the affected area and sample results to verify the area was remediated. The Operator immediately reported the spill and began remediation. MSDS sheets provided.

2393114 Primary Facility 2015-07-27 Incident- Response to Accident or Event Pending Inspection in response to a one gallon of flow back water spill reported to the Department on 7/24/2015. The Operator is in compliance with the current spill policy regarding spill less than a barrel. The Operator indicated that spilled substance was vacuumed and disposed of, however at the time of inspection WQS Steingrabe observed a stained area on the gravel well pad in the vicinity of the spill. The operator indicated that they have been unable to excavate that area due to the extreme pressure associated with the wells production. WQS Steingrabe suggested the Operator excavated the affected material as soon as safely possible.
2412457 Primary Facility 2015-09-22 Administrative/File Review In Compliance with Policy Admin. inspection in response to the Operator self-reporting a 1-2 gallon brine release to the gravel well pad. On Sept. 21, the Department received the Operators response to the release. The Operators visually clean up the affected material on 9/2/2015. The Operator is in compliance with the most current Department spill policy.
