  • Access road entrance requires additional rock, and a sweeper truck may be necessary due to a large amount of mud being carried out onto
   Windows road from the access road. 
  • E&S plan states the project will include the construction of +/- 1400 feet of access road. Actual distance of road constructed was +/- 2500
   feet.  Please change this in the E&S plan. 
   Please address these concerns and contact this office upon completion.

Lined pit appeared sound, and had adequate freeboard. Additional rock has been applied to the access road entrance. Appropriate changes were made to the E&S plan in relation to the access road. The above changes were requested through compliance assistance in an inspection report dated 5/28/09. Site is now in compliance.

Access road stable with no signs of accelerated erosion. Access road entrance stabilized with rock. BMP's in place in accordance with E&S plan and well maintained. Pits have been back-filled, and site regraded to contour. Site has been seeded and mulched. No violations found.

2098192 2012-08-21 No Violations Noted Inspection on request of operator. Operator received a complaint regarding access road and wanted department thoughts on condition of access road. Access road is gated. Initial access road entrance is shared to homeowners driveway, minor rutting exhibited department recommended that operator add additional rock to minimize rutting. Access road leading to well site(s) appears stable, again some minor rutting that operator agreed to add rock. No accelerated erosion or sedimentation issues note don well site or access road. Complainant has not yet filed a formal complaint with department, at this time operator received complaint and is trying to address as appropriate. Operator agreed to documents "changes" via photographs. No violations noted at the time of thin inspection.
