0834462502 2018-11-07 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. I found the well equipped with 4.5 inch diameter casing and 1.9 inch diameter tubing with the production valves open. It is setup to be produced by means of a pumping unit with rods and an electric motor. It will be noted that standing fluid around the wellhead and separator (outside of the dike) tested positive for chlorides with a High Range Quantab resulting in approximately 1,000 parts per million. The production facility consists of (2) 100 barrel steel tanks within an earthen dike, which dont meet the minimum spacing separation requirements. Additionally, an orifice sales meter with a current chart is present near the separator, outside of the dike area. The tank and wellhead are properly identified and the production records are current. I spoke with Sam Johnson with Heinrich Enterprises, on 11/7/2018, and explained the issues that need to be addressed. He indicated that he will be working to remediate the issues. The preliminary restoration does not pass on this day, and a follow-up inspection will be conducted.

1379950972 2013-09-23 Preliminary Restoration No Violations Status check inspection on the preliminary restoration found no restoration work done at the well site other than the drilling pits being closed, the frack tanks are still on site, the well is in production with 4.5 casing- 1.5 tubing and pumping rods with the production valves open. The separator is located on the drill site and a 100 barrel tank is located near the township road. A call was made to Brian Chavez on this day and He was asked to restore the site properly and post proper well identification, we discussed the presence of brine water inside the tank battery. The restoration was not passed on this day.
