| I inspected this producing unit as part of regular site check and inspection activity. The well was completed on 7/21/2007. Elements of the well consist of a production wellhead with standard valve assembly and pumpjack system located on the soccer/football practice field adjacent to the Shaker Heights Middle School. The wellhead is located in a closed and locked chain-link enclosure. There are no signs of leakage or problems at the wellhead.  The aboveground storage tank battery is located west of the wellhead, adjacent to the front parking for the school. The battery is comprised of 2 steel 100-barrel production tanks, and horizontal separator. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged and locked. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. The area of the well is generally level. Surface drainage is controlled by municipal storm sewers and inlets.  I did not observe any signs of gully formation or washouts. Access to the tank battery is afforded by entry to the Middle School from Shaker Blvd. There is no designated access routes or roadway for the well. Beyond the area cleared for the wellhead and tank battery, the area is occupied by the Shaker Heights Middle School and grounds and single family residential beyond in all directions. The general landscaping and appearance of the well and battery is good and well maintained.

| I inspected this well as part of routine inspection activity. The wellhead pump-jack unit and associated tank battery are located on the grounds of a public school facility. At the time of this inspection, I observed the following: the well and tank battery board-on-board enclosures were locked and secure. Fittings and connections were secure and free from signs of leakage or problems. The secondary containment was intact. I walked the areas surrounding the well and battery and found the natural hardscape and greenery to be free from signs of impact.

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1262882864 2009-11-12 Production Wells No Violations no violation.
1422453266 2014-06-19 UrbanProduction Wells No Violations I inspected this producing unit as part of regular site check and inspection activity.

2034813569 2017-08-31 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity.
