I went into the office to meet with Tom Chec Drilling Consultant for Hilcorp to see when the surface job was coming up. The drilling permit stated that we needed to set approx. 870 of 13 3/8 conductor or 50 below the Berea and cemented to surface. Tom looked at his drilling program and it stated that he needed to set 1440 of 13 3/8 conductor, he called his supervisor in Houston Texas, they had made a mistake in recording there evelation numbers.

At the location we met with Hugh Gregorcky, Construction Superintendent for Hilcorp, David E. Mostoller and B. J. Batterson with RETTEW Environmental Services. We looked over the construction plans and drilling pad location plans. This area is possible a fill from the mining of limestone. This is an Urban Location.
1353345509 2012-11-18 Drill / Deepen / Reopen No Violations Pro- Energy drilled a 26 hole to a depth of 149, They ran a total of 80 of 20 94# conductor casing. Nabors Completion Services was on location to cement the conductor with 480 sacks of class A cement. Good cement returns to surface.
1354399090 2012-12-01 Urban Drill / Deepen / Reopen No Violations I was onsite for the cementing of the surface casing. Franks casing crew was having issues getting the casing into the hole, they also encountered several bad joints that needed to be removed and replaced. The rig circulated on the hole for .5 hours and did not see any gains or losses during circulation. A total of 49.5 barrels of cement came back to surface during the job.

1356106561 2012-12-08 Drill / Deepen / Reopen No Violations On 12/8 2012, I received a phone call at 20:00 hour from Tom Cheh Drilling Consultant for Hilcorp Energy on H&P Drilling Rig 225, that they had to switch from air to fluid they had gas reading in 5900/6000 they had a flair, they used 10.2# mud to kill the flair. The depth was around 5700 ( Clinton formation ). They will continue to drill on fluid to Total Depth 8000.

1356550129 2012-12-26 Drill / Deepen / Reopen No Violations I was onsite to check the status of the drilling of the drilling rig. They are currently pulling out of the hole to change bits at their current depth of 13338 feet. They are intending to continue drilling tonight to a total depth of around 17000 feet. They are expecting production casing to be coming into tomorrow on semi truck as long as the weather does not limit travel. There are no violations at this time.
1357922687 2013-01-09 Plug / Plug Back No Violations I was onsite to witness the second kick off plug set on this well. A Baker Hughes inflatable packer was set at 7487 to allow for a 500 cement kickoff plug to be set on top of it. There were issues with unlatching from the packer because they could not get enough torque on the packer to properly unlatch. A wire line truck with a chemical cutter was being called out for the job but they were able to unlatch on a final attempt. They pumped 25 barrels of Tuned Spacer, 226 sacks of Densecem at 16.8 lb/gal and 1.02 yield. They followed this with 5 barrels of Tuned Spacer and displaced with 109 barrels of freshwater.

We talked about the 5.5 production casing that they will be running and their cementing procedure. I inspected the location and all containment is in place. The drilling area is clear of spills. The there are fire extinguisher around various locations of the drilling rig.
1359140475 2013-01-08 Drill / Deepen / Reopen No Violations When I arrived on location they were pulling their drill pipe out and getting ready to run the Weatherford Inflatable bridge plug. Bill Mitchell and I talked with the Weatherford Personal about the installation. They informed me that they would be ready in the evening. I instructed them that I would be on location at 6:00 pm.
1363720309 2013-03-08 Surface Facility Construction No Violations I was in the Uniontown Office, when Wendy Booth told me that she had received a compliant call from an anonymous caller, stating that someone in a large truck was dumping brine in a field by his house. When I called the person back he said that they were backing into a field an dumping. When I arrived at the location on Arrel Road east of Struthers Rd. I could see a water truck ( Haas Environmental Svc.) in the field. I spoke to Butch Peterson Superintendent for Southeast Horizontal Drilling, he said that they were moving fresh water boring fluid from the receiving side and taking it to the boring side and reusing it. This pipeline is for the Poland CLL1 1AH well. No violation at this time.

Halliburton Services installed their cement head and tryed break circulation. They pressured up to 5000 psi., shut down pulled 4 joints of production casing out and tried it again but could not break circulation at 5000 psi., Hilcorps Drilling Engineers instructed them to pull the 5.5 casing out, they pulled 16156 of 5.5 casing before getting stuck in the well bore. They used a chemical cutter to cut the casing at 7497 that left 1244 of 5.5 casing in the well bore. I notified Deputy Chief Robert Worstall and Steve Opritza of this, after a discussion with Hilcorp Energy they are gone back in the well bore and make a cleaning sweep before setting a Weatherford Inflatable bridge plug at 7497 and a 500 cement kick plug above that and build another kick off. There is a drawing of this in the well file.
1810388581 2013-01-08 Drill / Deepen / Reopen No Violations I arrived on location to witness the setting of the Weatherford Inflatable Packer and the cementing of a 500 plug. H&P Rig 225 ran drill pipe and tubing in the well to set the packer at 7497. The packer was to be set in three stages, starting with 500 psi and holding for 15 minutes, then with 800 psi and holding for 15 minutes, then the last with 1200 psi and holding 15 minutes and then evacuating the pressure. The packer would not set. This was tried several times. The decision was made to pull the drill pipe and packer back out of the well. They will call with there new plans.
1893672905 2015-09-28 Production Wells No Violations I performed a status check on the above location. Upon my inspection the well is shut in all values are shut the GPU has no pressure. At the central facility compressors are shut down. No noted violations at this time.
1916416709 2015-06-17 Production Wells No Violations I performed a status check on the above location. The well is in production. It is a constant flow system. ID signs are posted at the well head. All safety areas are marked and emergency shut downs are marked. No noted violations at this time. There is one central oil/water collection system with twelve 400 barrels tanks. There is also 2 compressors and various fluid knock out systems on this pad. At this point the gas enters into a midstream pipeline system. Preliminary Restoration is passed

2136307355 2013-08-26 UrbanProduction Wells No Violations I was onsite to perform an inspection on the urban production well. The wellhead was currently producing and the gas is being flowed to a production facility. Currently a compressor is onsite running to push the gas to the sales lines.
