A plug shall be placed in the heater tube to prevent further leakage as soon as possible. The well owner/operator shall submit a written plugging schedule to Todd Miller at 2207 Reiser Ave. SE, New Phila., OH 44663 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |
