-1018858628 2009-05-04 Received request from Tom Tomastik to check TAAD status on this well. Well is not equipped for annular disposal. Well is equipped with a 100 bbl tank and mechanical separator. 2 -2" lines are connected to the tubing. With valves (both closed) 1st line goes to tank (100bbl). 2nd line goes to mechanical separator. Cannot see annular space on well it is below grade. Well is equipped to supply house gas. 4.5" casing is connected for house gas with a 2" valve reduced to 1" and connected to a pressure regulator and a gas meter. House line is 1/2" black plastic.

-887887960 2013-11-20 This well or adjacent well battery do no have an identification sign. According to 1501:9-9-05(A)10/1509:23 the wellhead should have attached a sign with at least the following information: current well owner, lease name, well number, permit number, county, and an emergency telephone number. The owner shall install a sign at the wellhead with at least the information listed above. Called Jonathan Reese, well tender for JD Drilling to erect the sign. He will get to it when he is in the area checking on other wells. 9-9-05(A)10
1820796587 2014-05-14 Upon inspection, it was observed that there was a new identification placard posted at the well head which was legible and correct. This satisfies the requirements of the compliance notice.
