1367931740 2013-05-06 I received a call on 5/4/2013 at 9:15 pm from ODNR communications (614-799-9538) about a well in Champion that had been shot and was leaking fluid. I found the well belonged to Enervest and called their field supervisor at 216-570-8708, Larry Lower. He immediately dispatched a well tender to assess the situation and make repairs. Larry found the 1.5" tubing on the plunger lift unit had a small hole from a gun shot. Also, a coupling on the flow line had a mark from a gun shot. The well tender placed a clamp on the tubing to stop the leaking. On 5/5/2013, I received a call from Trumbull County 911 that the tubing was leaking again. I called Larry Lower and he dispatched the well tender. The well tender replaced the clamp with a new full circle clamp until further repairs could be made. My inspection on 5/6/2013 found the clamp holding. I noticed a 10' grassy area around the well that had been impacted from the leak. Enervest had peat sorb and absorbent pads in place. A Bobcat was dispatched to the location to remove all of the grass and soil impacted. Approximately 3 to 4 barrels of brine was released. I researched our database and found the well had not produced since 2006 under Belden and Blake Corporation. The tank battery contained one, 100 barrel production tank. The dike was weathered and did not meet containment requirements. The heat, treated separator was surrounded by debris from the resident. There was an identification sign posted on the production tank. The was no lease road to the tank battery. Larry Lower informed me that Enervest was working on contacting the landowner to get access to the tank battery and well. Enervest was issued a plugging permit on 3/6/2013. 9-1-02
