| Drilling was ongoing at the time of the inspection. Drill and frac pits located onsite at the time of the inspection and they had more than the required two feet of lined freeboard.  Noted that it was raining at the time of the inspection and that all erosion control BMP¿s appeared to be functioning properly.  No accelerated erosion & sedimentation was observed in relation to the access road or well-site at the time of the inspection.  Noted that it appeared that significant dirt moving and culvert replacement would have had to have been undertaken to make the existing stream crossing for Fagley Run capable of handling the truck traffic necessary for the development of wells located on the well pad.  At this time the Department is requesting that the operator forward documentation outlining what work, if any, was performed by the operator to make this stream crossing adequate for well development.  This documentation should be forwarded no later than 06/18/10.

| Drilling was ongoing at the time of the inspection.  Drill and frac pits located onsite at the time of the inspection and they had more than the required two feet of lined freeboard.  Noted that it was raining at the time of the inspection and that all erosion control BMP﾿s appeared to be functioning properly.

1886794 2010-05-17 No Violations Noted Drilling was ongoing at the time of the inspection. Drill and frac pits located onsite at the time of the inspection and they had more than the required two feet of lined freeboard. Noted that it was raining at the time of the inspection and that all erosion control BMP﾿s appeared to be functioning properly.
1888376 2010-06-01 No Violations Noted Drill rig was not onsite and drilling was not ongoing. Drill and frac pits located onsite at the time of the inspection and they had more than the required two feet of lined freeboard. Noted that all erosion control BMP﾿s appeared to be functioning properly. No accelerated erosion & sedimentation was observed in relation to the access road or well-site at the time of the inspection.
1898201 2010-07-14 No Violations Noted Drill rig onsite and drilling was ongoing for permit 005-30590. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. All erosion controls appeared to be functioning properly. Below grade pits located onsite had more than the required two feet of freeboard.
1903492 2010-07-13 No Violations Noted drilling @ 6700ft/E&S ok/ID&pit ok
1910595 2010-09-03 No Violations Noted Operator was setting up onsite to begin drilling out the plugs after completion of well stimulation. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. All erosion controls appeared to be functioning properly. Pits located onsite had more than the required two feet of freeboard.
1929885 2010-11-23 No Violations Noted Drilling appeared to have been completed at the time of the inspection. Well appeared to be equipped for production at the time of the inspection. Drill and frac pits still located onsite at the time of the inspection. Pits had more than the required two feet of lined freeboard and no evidence was observed to suggest that either of the pits were losing any fluids. No accelerated erosion & sedimentation was observed in relation to the site or access road.
1950030 2011-02-15 No Violations Noted Drilling appeared to have been completed at the time of the inspection. Well appeared to be equipped for production. Drill and frac pits still located onsite at the time of the inspection. Pits had more than the required two feet of lined freeboard and no evidence was observed to suggest that either of the pits were losing any fluids. No accelerated erosion & sedimentation was observed in relation to the site or access road. Due to winter weather and truck traffic the access road did require routine maintenance (i.e. re-rocking, ditches, culverts, and turn-outs cleaned).
1965635 2011-04-14 No Violations Noted Drilling appeared to have been completed at the time of the inspection.

2058570 2012-04-03 No Violations Noted Drilling was completed on 07/23/10, well appared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Both drill pit and a frac impoundment were located onsite. Both had more than the required two feet of lined freeboard. There was no accelerated erosion or sedimentation observsed in relation to the access road or well-site. All erosion controls ahd been implemented as outlined in the E&S plan included with the approved ESCGP.
2074096 2012-06-05 No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Site regrading activities were undeerway at the time of the inspection. Frac impoundments had been regraded, and the drill pit was in the process of being regraded.
2082764 2012-07-12 No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Drill pit and frac impoundment areas had been regraded, seeded & mulched. It was noted at the time of the inspection that the sediment trap had been left in place onsite. There was no observed accelerated erosion or sedimetation in relation to the access road or well-site.
2170537 2013-05-30 No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road. The site had been stabilized and the vegetative cover met the 70% uniform coverage criteria. It was noted that the sediment trap had not been removed and was still located onsite. Sediment trap appeared to be functioning properly.
