| During the investigation, I used  an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen ponded water inside the duck pond  type containment area where a pump was located and the secondary containment for the frac tanks. I also screened an area outside the containment area on the Well Pad close to where pump and filtration system are located.  Conductivity readings indicated over the limit  inside the duck type containment area and the area of ponded water on the Well Pad I mentioned above. The limit on the meter is 19.99 milliSiemens/cm.

The pump and filtration system as part of the alternative waste management practice at the site, has been re-located.  It is now staged next to the gas buster tanks, where the secondary containment for the frac tanks has been extended. The gas buster tanks have been re-positioned.  The small area of ponded water on the Well Pad has been removed.  The whitish precipitate/residue and piece of the secondary containment liner located between the frac tank containment and secondary containment in the vicinity of the wells has also been removed from the site.

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION_COMMENT
1957788 2011-03-15 No Violations Noted Comp man Jeff Myers H&P 320 Rig closed loop system

Drill another 300' then ready for inter. casing Drillers log checked ok: the Solanick 5H was drilled on air and there was no water, gas I checked the cellar of the 5H, no bubbles. Holes in plastic containment- they are working on it now. 20" cond. @ 70'RKB 68 sx of Cl A RKB is 30' above ground level 13 3/8" surf @ 718"RKB 626 sx Cl A with 45 bbl to pit Avg wt is 15.4 ppg

1958046 2011-03-16 No Violations Noted Was on site to watch cement job.

9 5/8" set @ 1714rkb with 594 sx Cl A 21 BBLs to Surf. avg density= 15.2 ppg

1960282 2011-03-25 No Violations Noted proposed depth is 7500' on air, just moved back on from 2H this morning, didn,t go near rig- testing BOP to 5000 psi
1961819 2011-03-30 No Violations Noted Mud logger not here, didn't check log book
1964432 2011-04-07 Pending H&P Flex Rig 320 @ 5593' w/ oil based mud

New company man on site - Wayne Simon didn't know about dtrillers log- not up to date secondary containment- crushed edges and holes

1964801 2011-04-11 No Violations Noted checked drillers log- good

working on site- putting rock in ruts & muddy areas, changing secondary containment, new plastic, rebuilding berms.

1966447 2011-04-18 No Violations Noted drillers log checked at TD - 13460'

Discussed cleaning up containment in back. Probably cement prod string late tomorrow.

1967734 2011-04-22 No Violations Noted done drilling, just reporting cement info.

5 1/2" prod. set @ 13458' w/ 1360sx of 13.5 ppg & 1536 sx of 14.5 ppg to surf, 57 bbl to pit top of tail @ 6465'

1990336 2011-07-15 Violation(s) Noted No well identification. 616171 201TAG - Failure to install, in a permanent manner, the permit number on a completed well
2002575 2011-09-12 Pending Cellars full of water. Didn't see well tags? Extreme bubbling in 2 wells w/ lesser amount in 3rd. Took video. May be from prod csg, but can't tell. Will need to get pump truck and identify if it is gas and where it is coming from.
2003154 2011-09-14 No Violations Noted Cellars were full of water - bubbling so follow up - checking where bubbles were coming from. In all cases - from 5 1/2 X 9 5/8" annulus vent. No bubbles from 9 5/8 X 13 3/8" or 13 3/8 X 20" or outside 20"
2083738 2012-07-10 No Violations Noted During the investigation, I used an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen ponded water inside the duck pond type containment area where a pump was located and the secondary containment for the frac tanks.
2085662 2012-07-11 No Violations Noted During the investigation, I used an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen a small area of ponded water outside the containment area on the Well Pad close to where pump and filtration system are located. Conductivity readings indicated over the limit with the limit on the meter being 19.99 milliSiemens/cm. During the inspection a vaccuum truck mobilized to the site and began to remove the ponded water on the Well Pad from the site. The Department recommended that Carrizo remove the whitish precipitate/residue on the Well Pad, repair any areas of the secondary contaiment liner experiencing compromised integrity and re-position the gas buster tanks prior to the commencement of flowback.

Whitish precipitate/residue was observable on the surface of the ground of the Well Pad as evident during the 7/10/12 inspection. Compromised liner integrity was observed on the Well Pad and inside the secondary containment area for the frac tanks.

2086417 2012-07-16 No Violations Noted During the inspection, Carrizo was in the process of gas flaring and flowback and filtration of the wells at the site.

2164798 2013-05-01 No Violations Noted Related to Mazzara Incident: 2162665
2164803 2013-05-07 Pending
