1953539 2011-03-01 No Violations Noted Site inspection for location of potential gas well within 200 ft of blg/water supply. Site located 140 ft from building in pasture. Wind direction at the time of inspection was SW to NE. DEP request well location has pit on the north/northeast portion of pad ﾿ leeward side of site opposite from building. BOP installed on surface casing or intermediate string through completion of drilling.
2024538 2011-12-05 No Violations Noted Well sign present. E&S controls were implemented. At the time of inspection, no E&S or IW issues. Pit was dug.
2041218 2012-01-30 No Violations Noted Site visit was due to spud notification.

Well was drilled and appeared to be in production. There was a house 45 yards (via range finder) from the well head. A waiver is necessary to drill this close to a building. The site plans were inaccessible due to moisture. According to Paul Wyrwas PA DEP Inspector, waiver was applied for and maintained. Filter fabric fence was installed and working properly, although it was installed nearly to the building. Roll off boxes were on site and contained liner and what appeared to be possibly cuttings. These should be removed ASAP. Both the site ID sign and the well tag had invalid permit numbers. PA DEP Water Quality Specialist APril Weiland was present during inspection.

2152682 2013-03-22 No Violations Noted At time of inspection, well in production. Brine tank sound. Access road adequately rocked, stable.

Well tag present, legible; however, name of well incorrectly spelled and permit number also incorrect. Production equipment on rocked pad, stable. Surrounding well site adequately vegetated and permanently stabilized. Department recommends Operator continue to monitor and maintain AR and well site as necessary. Slight gas odor noted.

UPDATE: 4/2/2013 Department notified Production Administrator about issues with well tag via phone.

UPDATE: 4/3/2013 Production Administrator contacted Department to notify that well tag had been replaced that morning.
