Noted that it was raining at the time of the inspection and that all erosion control BMP¿s appeared to be functioning properly. No accelerated erosion & sedimentation was observed in relation to the access road or well-site at the time of the inspection. Noted that it appeared that significant dirt moving and culvert replacement would have had to have been undertaken to make the existing stream crossing for Fagley Run capable of handling the truck traffic necessary for the development of wells located on the well pad. At this time the Department is requesting that the operator forward documentation outlining what work, if any, was performed by the operator to make this stream crossing adequate for well development. This documentation should be forwarded no later than 06/18/10.

All erosion controls ahd been implemented as outlined in the E&S plan included with the approved ESCGP.
