Mr. Rader understood this was to be addressed as procedure concerns on all Antero sites in general.

There are also contractors on site emptying the earthen pit and preparing to close it in. The area around the site has vegetation established and the drilling equipment has been moved off  with the well currently in production. The tank battery has been erected on the site pad. 8 - 400 barrel storage tanks are on site.

| Follow up inspection to complaint # 4510 forwarded over from Aaron Wolfe with the Ohio EPA, The photographs sent to the EPA by Ruth Partin were reviewed by my Supervisor Rocky King and Myself and along with the EPA it was found no violations were present in the photos, The request by Ruth Partin to disclose the nature of the production equipment on location will be for site safety reasons forwarded to Antero's public relations department for answering, The EPA provided only a email address and failed to document a time or day of contact as well as any personal contact information for Ruth Partin. My on site inspection found my recent request after my last inspection on 10/02/2013 to post proper identification met, All 3 wells on location are in production and a Myers tank truck was on location withdrawing processed water with proper safety grounding and spill containment in place while loading, The Myers UIC number was also properly posted on the tank truck.

| While on site at the Carpenter drill pad I was notified by the safety man that they had a small fire on the Rubel production pad, I was on site within 6 minuets and no fire was observed and it was discovered that a hammer union on a excess gas burner had become loose causing a small amount of gas to escape and flash when the burner ignited, The hammer union was tightened and the company agreed to install safety devices between the production tanks and the safety burners as not to allow any liquids to migrate from the tanks to the burners of allow any potential hazard to back feed into the production tanks through the gas escape ventilation system.

On this day Inspector Eric Conrad and myself did a calculated approximation of the gas production and found the 2 wells involved in the complaint to be meeting the minimum standards of gas production. Our findings were as follows, The chart is a 30 day 100# chart on a 500# meter with a approximate line pressure of 3 psi =.75 mcf or 750 cubic feet of gas produced per day x 365 days in a calendar year = 273 mcf of produced gas per year meeting the State minimum of 100 mcf of gas per year per well.

1366753362 2013-04-23 Status check inspection of the current drilling progress found the current drilling depth to be 3201' and a wire line company currently doing a borehole deviation survey. The next casing string to be set will be the 9.625 intermediate casing string at 6950'. I also received the 13.375 intermediate casing string paperwork records from 04/20/2013.
1367242696 2013-04-28 I received phone notification and was in phone contact with the company man on site during the running and setting of the 9.625 casing string and casing cement job. The current well bore stage drilling depth was reached at 6,951' using a 12.250 drill bit and 6,912' of 9.625 casing was ran and cemented with a cement top @ 500'.

The well is has been completed and the testing and completion process is finished and the well is hooked up awaiting the completion of the on site processing facility and gas transportation lines in area to be complete.
1380726541 2013-10-02 Status check inspection on the well and location, On this day I spoke with Antero's safety coordinator and asked that each well head have proper identification and proper identification be placed at the entry point of the location. There are also contractors on site emptying the earthen pit and preparing to close it in. The area around the site has vegetation established and the drilling equipment has been moved off with the well currently in production. The tank battery has been erected on the site pad. 8 - 400 barrel storage tanks are on site.
1382911916 2013-10-27 Follow up inspection to complaint # 4510 forwarded over from Aaron Wolfe with the Ohio EPA, The photographs sent to the EPA by Ruth Partin were reviewed by my Supervisor Rocky King and Myself and along with the EPA it was found no violations were present in the photos, The request by Ruth Partin to disclose the nature of the production equipment on location will be for site safety reasons forwarded to Antero's public relations department for answering, The EPA provided only a email address and failed to document a time or day of contact as well as any personal contact information for Ruth Partin. My on site inspection found my recent request after my last inspection on 10/02/2013 to post proper identification met, All 3 wells on location are in production and a Myers tank truck was on location withdrawing processed water with proper safety grounding and spill containment in place while loading, The Myers UIC number was also properly posted on the tank truck.
1384561900 2013-11-15 While on site at the Carpenter drill pad I was notified by the safety man that they had a small fire on the Rubel production pad, I was on site within 6 minuets and no fire was observed and it was discovered that a hammer union on a excess gas burner had become loose causing a small amount of gas to escape and flash when the burner ignited, The hammer union was tightened and the company agreed to install safety devices between the production tanks and the safety burners as not to allow any liquids to migrate from the tanks to the burners of allow any potential hazard to back feed into the production tanks through the gas escape ventilation system.

-482809979 2014-03-13 Inspector Eric Conrad and myself met on site with Antero's production supervisor Daren Rader discussing several site concerns listed as follows, site entry point identification, proper site soil compaction over the recently installed production flow lines, procedures for identifying temporary storage tanks with proper liquid content identification, general procedure safety during the completion drill out process, proper mapping and identification of buried underground production lines, proper containment under all fluid vessels on site. Mr. Rader understood this was to be addressed as procedure concerns on all Antero sites in general.

742388077 2013-10-29 Follow up review to complaint # 4517. On this day Inspector Eric Conrad and myself did a calculated approximation of the gas production and found the 2 wells involved in the complaint to be meeting the minimum standards of gas production. Our findings were as follows, The chart is a 30 day 100# chart on a 500# meter with a approximate line pressure of 3 psi =.75 mcf or 750 cubic feet of gas produced per day x 365 days in a calendar year = 273 mcf of produced gas per year meeting the State minimum of 100 mcf of gas per year per well.
953108746 2013-04-14 Notification was made to the inspector that the initial conductor cement job was communicating to surface when they started to drill the surface bore hole. The conductor was re cemented using 704 sks of Class A 2% calc, 1/4# flake -15.6 # 1.18 yield cement with returns to surface.
