1826727 2009-09-08 No Violations Noted Inspection conducted in response to recent complaint, that heavy sediments are coming from access road and well pad because BMPs are not installed or are installed improperly. Inspection revealed the following:

1) multiple loads of gravel were being hauled onto site to surface access road. road was gravelled to about 300' beyond first existing gas well. grader and roller following closely behind gravel trucks. 2) road and cut sides of road are compacted with no signs of erosion. sloped sides of road are bare 3) culverts are in place in multiple spots along road. Currently exposed soils are at the inlets and outlets of these culverts 4) Site ID sign and E&S plan are at entrance off of S.R. 3016 company asked to ensure proper installation of BMPs

1831582 2009-09-29 De minimus Violations Noted -Access road complete and stabilized. all disturbed areas along road have been seeded and mulched. Filter fence installed where applicable.

-top hole has been drilled. Construction company on-site working on pond A. Drilling surface and drilling cuttings pits are complete and stabilized -pit and pondis lined with no punctures or tears in visible surface. there is greater than 2' of freeboard. -Super silt fence is installed along northern edge of site. joints are done improperly, causing small section (about midway of pit) to become untowed and fold back on itself. most likely from wind. there is no sign of E&S issue resulting from this joint failure. -Site ID sign and E&S plan are at entrance off of S.R. 3016 -cellar filled with water and gas is bubbling out at a constant rate. Referred to Bruce Jankura -some E&S controls are not installed according to E&S manual and will need to be addressed


1833541 2009-10-06 No Violations Noted -Met Brian Welch, Construction Specialist on site to approve changes on E&S plan.

The following changes have been noted: -ponds B&C will not be constructed. an approved withdrawal point will allow Pond A to be continuosly filled when needed. the farthest spoil storage will also be unnecessary -filter fence has been adjusted to cover new site dimensions -cuttings pit is two compartments instead of three -ditch runs between eastern edge of pond A and Western edge of pad These changes significantly decrease the proposed amount of Earth disturbance and decrease the risk of Erosion and Sedimentation -previosly noted deficiencies were brought to Mr. Welch's attention and should be corrected

Site Sign and E&S plan were at the end of access road. All E&S controls were in plae and functioning. Rig was in process of ringing down and preparing to move out.

1863436 2009-02-17 No Violations Noted -recieved complaint that multiple water trucks marked residual waste were dumping their contents into the Clearfield Creek.

-Inspected alleged dump site and Hurd well with Travis Walker. We found that the site is on the Northwest shore of Faunce Rd bridge, crossing the Clearfield Creek in Faunce. It is an SRBC approved water withdrawal point for EQT (docket approval #20081202). There were no wastes being dumped into Clearfield Creek. -at the well pad multiple trucks were dropping their loads into the fresh water impoundment. the site was snow covered and a full E&S inspection could not be conducted. There were no obvious signs of E&S issues and both the reserve pit and impoundment were lined with no visble punctures or tears. both were within 2ft of freeboard -Travis Walker has one open violation on site and will oversee the closing of that violation

Drilling is completed, and the well is shut in. Cellar is filled with water with a layer of ice on the top. Well head is leaking from somewhere below the surface of the water. Gas is bubbling up through the water all around the wellhead. There was no discernable smell of natural gas, but the gas could be heard bubbling. Pictures and a video were taken of the well head leak. A spill was noted between the well head and the pit. This spill appears to either be drill cuttings, or drilling mud. Spill will be turned over to WQS Sam Jones.

The spill that was noted in inspecion # 1860506, dated 1/22/10, has been cleared from the well pad and placed into the impoundment pit. Area that was cleared, was then filled in with gravel.

-They had the spill, that Travis Walker and Sam Jones previously noted in an NOV, cleaned up -all E&S controls were still implemented andworking adequately despite the snow -both pit and impoundment had adequate freeboard and liners appeared sound -no issues remain on-site at this time
