| Glenn Davis Well Service drove packer which is on 2-3/8 inch tubing to 5750 ft and now we are waiting on Appalachian Well service to shoot off the tubing so that the tubing can be used to set the bottom plug and then Appalachian Well can shoot again at 4800 ft out the bottom of the 2-3/8 and shoot thru the 4-1/2" inch diameter casing. 15 shots were used to perf the casing so that a second plug could be set before pulling up on the casing. The well has H2S and there is a hole in the casing so I do not want to try and rip it before I can perf and set another plug. Formation Cementing Service set both plugs thru 2-3/8" inch diameter tubing. Next Davis Well will pull out all of the tubing so they can try and rip the 4-1/2' inch diameter casing. Then all of the casing needs pulled and tubing ran back to plug the remaining plugs to surface.

| I am on site today with Glenn Davis Plugging rig. The had trouble getting the rig running because it was -14 last night and is only -8 now. After the rig was started they tried to free up the packer which is on tubing inside 4.5" inch diameter casing. They could not free up the packer to pull it so I advised that they drive it down past the perfs and call a shooter out to shoot off the tubing and while on site shoot a hole at 4800 ft to cover the Newburg which has H2S.  It was not covered during the production casing cement job and needs to be covered. The 4.5" inch casing is full of holes and my part and can not be used to set any plugs. Job will be on Friday if shot truck and cementers can be on site.

| I am here to check on the final restoration and found that the well site has a pile of dirt which needs graded and the production tanks and other producing equipment needs to be removed. Found that the well is the landowners also all of the equipment so the restoration final is approved.

| I was contacted that Glenn Davis was moving in today and would be plugging this well. I did not have the permit number just a lease name and did not find the rig. I contacted Inspector Grimm who also talked with the operator. While looking for this well I also needed to meet with Inspector Grimm to review follow up inspections which he covered while I was off.
