| I was on location to follow up on complaint #4403. The well and tank battery have recently been worked over. A new dike, 100bbl production tank and separator have been installed where the existing tank battery stood. The lease road has been stabilized with several loads of stone and ceramic slag. A new chart has been installed in the chart box on 11-12-14 and shows that the well is producing. I have contacted PDL Services and they told me that the 2013 production reports are currently being submitted to the Division of Oil & Gas as required.
Issues for PDL Services Inc to address include; access road must be graded, the belts replaced on the pumping unit, bull plugs installed on the tubing and 4.5 inch lines on the wellhead, oil contamination around wellhead cleaned up to include removal of contaminated soil and neutralizing, vegetation removed from the tank battery dike, the western side of the dike built up to prevent further erosion, and leaking fittings on the separator fixed.  This well must be placed into production or plugged.

| I received an email from Dave Claus regarding a call he received from Jodi Carnahan on November 9, 2012.  I inspected the Direnzo-Fimble Unit #2 well (34151234030000) on November 14, 2012; I was accompanied by Tom Hill.  My inspection showed the lease road was deeply rutted and needs to be graded.  The domestic gas line is not buried and runs parallel to the lease road.  The line has several segments that are damaged.  The wellhead includes an electric powered pumping unit.  The belts on the pumping unit are burned off so the pumping unit is not functional.  The rods are still polished, indicating recent activity.  The tubing and 4.5 inch casing valves do not have bull plugs installed and there are no pressure gauges installed.  There is oil contamination surrounding the wellhead covering an area approximately 60 square feet.  There is an operator sign attached to the electrical box at the wellhead.  The operator reported production of 6bbl of oil and 331 MCF of gas in 2010.  There is no production posted for 2011.  The tank battery consists of one 210 bbl stock tank and one mechanical separator.  Fittings on the separator are leaking.  A quarter inch restrictor plate is installed to the sales line.  The 210 bbl stock tank has the proper operator sign attached.  There is a chart installed with an installation date of November 13, 2012, showing 90 psi on the sales line and no gas production.  The dike is overgrown with vegetation.  The western side of the dike is eroded due to run off to the streamThe property owner, Sam Sluss, met with us at the wellhead and said he hasn
