| I was on location with ODNR Inspector John Fleming to monitor clean up procedures on the tank battery. The stock tank had overflowed, contaminating the soil and gravel inside of the dike with crude oil. No contamination had escaped the dike. Wells Quality Excavating was conducting the clean up. They were removing all of the contaminated soil and gravel to be taken back to their facility to be land farmed. | | I was on location with ODNR Inspector John Fleming to monitor clean up procedures on the tank battery. | On 6/4/2012, I received a phone call from a Cindy Lawrence, she was calling on behave of her uncle about the Hunter #1 well on his property, she stated that it was in need of some repairs. Upon my inspection on 6/6/2012, I noticed that the 100 barrel production tank had ran over and also the in ground water tank had also ran over. The gravel is stained and contaminated, the tank needs to be removed and the gravel replaced inside the tank battery , the tank needs to be washed down and painted, all oil and water stayed inside of dike. Upon inspection of the well head the fence needs to be replaced and a leak repaired at the well head, There is one 100 barrel production tank. One vertical separator and one in ground concrete water tank. I called Larry Hanking the well tender for J. R Smail and told him to get this cleaned up as soon as possible, I also told him to call me when he starts the clean up. | | On 6/4/2012, I received a phone call from a Cindy Lawrence, she was calling on behave of her uncle about the Hunter #1 well on his property, she stated that it was in need of some repairs. | On 6/8/2012, Wells Excavating was on the Hunter #1 location to start the clean up. They removed 8 to 10 yards of dry stained gravel/soil and the tank was washed down. They will be back to put new stone inside of the tank battery and install a new ID sign, with the permit number and emergency number, on the 100 barrel tank. | | On 6/8/2012, Wells Excavating was on the Hunter #1 location to start the clean up. | I performed a random field inspection. I GPS'd both the tank battery and the wellhead for the first time. The ID sign was posted and the tank battery was in good shape. I found no violations at this time. | | I performed a random field inspection. I GPS'd both the tank battery and the wellhead for the first time. Upon my inspection, on 6/6/2012, I noticed that the 100 barrel tank and the in-ground brine tank had over flowed, but the released fluids had stayed within the diked area. I also inspected the well head, which is a plunger lift system. There was a small oil stain on the ground and the fence needs to be repaired. I called Larry Hankins, the well tender for J. R. Smail, and told him to get this cleaned up as soon as possible and call me when he starts the clean up. | ||

-1748313501 | 2012-06-06 | On 6/4/2012, I received a phone call from Cindy Lawrence. She was calling on behalf of her uncle about the Hunter #1 well on his property. She stated that it was in need of some repairs. |
