| On 6/14/2012, Wells Excavating was on location and put 10 tons of lime stone inside of the tank battery and cut the weeds. At this time the clean up is complete. They will be putting a new  identification sign on the tank. I will follow up on this.

| On 6/4/2012, I received a phone call from Cindy Lawrence.  She was calling on behalf of her uncle about the Hunter #1 well on his property. She stated that it was in need of some repairs. Upon my inspection, on 6/6/2012, I noticed that the 100 barrel tank and the in-ground brine tank had over flowed, but the released fluids had stayed within the diked area. I also inspected the well head, which is a plunger lift system. There was  a small oil stain on the ground and the fence needs to be repaired. I called Larry Hankins, the well tender for J. R. Smail, and told him to get this cleaned up as soon as possible and call me when he starts the clean up.

| I was on location with ODNR Inspector John Fleming to monitor clean up procedures on the tank battery. The stock tank had overflowed, contaminating the soil and gravel inside of the dike with crude oil. No contamination had escaped the dike. Wells Quality Excavating was conducting the clean up. They were removing all of the contaminated soil and gravel to be taken back to their facility to be land farmed.

| On 6/8/2012, Wells Excavating was on the Hunter #1 location to start the clean up. They removed 8 to 10 yards of dry stained gravel/soil and the tank was washed down. | I performed a random field inspection. I GPS'd both the tank battery and the wellhead for the first time. The ID sign was posted and the tank battery was in good shape.  I found no violations at this time.

-1748313501 2012-06-06 On 6/4/2012, I received a phone call from Cindy Lawrence.
