| On the day of my inspection I witnessed that Sidewinder Drilling Rig #63 drilled a 17.5 inch diameter hole to the depth of 1300 feet and set 1300 feet of 13.375 inch J-55 54.5#/ft. casing. Halliburton was on site and gained circulation with 150bbls of fresh water then pumped 50bbls of gel, 5bbls of fresh water and cemented casing with 1140 sacks of Class A cement with a 15.6ppg density and a yield of 1.21. Displaced cement with 194bbls of fresh water. Good returns of 60bbls of cement, plug was landed at 625psi and pressured up to 1150psi for 5 minutes and the floats held. Cement stayed to surface.

| On the day of my inspection I witnessed that flow back has been completed and the well is online and equipped with a production tree with valves in the open position. Production equipment included a sand trap, a GP Unit, an oil stabilizer unit, a vapor recovery system and a total of 15-400bbl tanks all within containment. The site passed preliminary restoration according to ORC 1509.072A.
