| I was on location to perform an inspection of this well regarding complaint #4668. During my inspection I found a graveled lease road that is used as a residential driveway. The tank battery consists of two empty 210 bbls production tanks and a vertical separator, all within an earthen dike. The tank battery and well have an I.D. sign present but it is not legible. The pump jack well does not have the required shroud around the drive belts and electric motor. There is a house gas connection on the well but it is not currently functioning. I have contacted the operator and informed them of the issues. This well is currently on the G&O Resources plugging list when plugging funds are available. The only production shown for this well since 2009 is 31 barrels of oil produced in 2010. This well shall be plugged with 180 days as required by ORC 1509.12B.

The plugging of the well and the plug schedule was discussed and it was decided plugs would be set through tubing due to concerns with 4.5 diameter production casing integrity. They will pull rods today and prepare to cement tomorrow 10-15-2014.

The power poles will be pulled from the ground and left on location for a contractor to remove. The concrete pad for the separator will be removed and the lease road up to the driveway will be graded and seeded. The location will be graded level, rocks removed and seeded so that the yard can be brought back to a condition that will allow for it to be mowed with a lawnmower. This work will begin within a week and at when the work completed final restoration can be passed.

The location was not graded, the flow lines are still present, the brine vault is still present in the former tank battery and the power line poles to the pumpjack have not been removed.
