| On the day of my inspection I witnessed the first stage of a two stage cement job. Wildcat Drilling had drilled an 11.625" hole on fluid to 5933'. They ran 8.625" casing equipped with a DV tool. Universal broke circulation and pumped 600 sacks of cement. Good circulation was maintained throughout the job.

| This report correlates with complaint #4667. I received a call from Ginger Maple with concerns that the new injection well in Coshocton was causing fluid to bubble out of her yard. The injection well in question is approximately 8.8 miles from Mrs. Maple's home. I met her at her home to conduct my inspection. I found that there was a steady flow of water from the yard. I also observed periodic bubbling from the ground where the water was pooled. They had dug a small hole at the source of the water and a small ditch to divert the water away from the yard. I conducted a field test of the water with a Quantab and I sampled the air from the bubbles with a hand held gas detector. The results were negative for chlorides and negative for CH4 or H2S. I also tasted the water and found it to be fresh. I concluded that the flow of water from the yard was not related to this injection well.

1383150553 2013-10-30 On the day of my inspection I witnessed the first stage of a two stage cement job. Wildcat Drilling had drilled an 11.625 hole on fluid to 5933'. They ran 8.625 casing equipped with a DV tool. Universal broke circulation and pumped 600 sacks of cement. Good circulation was maintained throughout the job.
1383324727 2013-11-01 On site to attempt to pressure casing, arrived on site and rig crew loaded casing with 15 bbl fresh water and then DB Energy Management used there solvent truck to apply pressure and he pumped additional 7 bbl of water with a pressure reading of 180 psi maximum so at that point decision was made to trip in hole and drill out stage tool and run bit to bottom and then set bridge plug in bottom of casing and load hole again and then pressure test casing.
1383525462 2013-11-01 On site to witness Appalachian Well Surveys run 8 retrievable bridge plug inside of casing and set it at 5875' and then rig crew loaded casing and we pressured tested casing@1500# and pressure dropped to 1485# during the 15 minute test which was a 1% decline.

Note: Appalachian had to make two runs with collar locator log due to faulty equipment.

Producers pumped 7.5 bpm@800 psi and once pump truck shut down it took six minutes for pressure to drop to 0 pressure.

1397748319 2014-04-17 This report correlates with complaint #4667. I received a call from Ginger Maple with concerns that the new injection well in Coshocton was causing fluid to bubble out of her yard. The injection well in question is approximately 8.8 miles from Mrs. Maple's home. I met her at her home to conduct my inspection. I found that there was a steady flow of water from the yard. I also observed periodic bubbling from the ground where the water was pooled. They had dug a small hole at the source of the water and a small ditch to divert the water away from the yard. I conducted a field test of the water with a Quantab and I sampled the air from the bubbles with a hand held gas detector. The results were negative for chlorides and negative for CH4 or H2S. I also tasted the water and found it to be fresh. I concluded that the flow of water from the yard was not related to this injection well.
