| Met with Cindy Vandyke on well that was drilled and was being plugged back with a coil tubing unit. The equipment was not able to make it in to location. Nabors was cementing company. They were on site and Waco rig was getting ready to run pipe in to set plugs.

| Steve Bumgardner, consultant, called to say rig TD'd this morning at 7:30 at 10,220.  Will start logging well tonight, then go back in and do side-wall coring.  They may also do some formation testing if warranted.  If not, will start plugging back on Friday or Sat.

| My arrival found rig going in hole with 13 3/8" casing.  I was unable to witness conductor cementing but had been to rig before drilling started and felt they had read their permit and pointed out to them when inspectors were to be notified.  After the cement crew had cementing head on and trucks hooked up, ready to pump cement, I realized they had not met permit conditions which called for 500' of surface casing - they had only run 336' after drilling 17.5" hole to 361They also did not set enough conductor, setting only 30' when the permit called for 100'Again stressed the importance of reading their permit and let them go ahead and cement this lesser amount of surface casing, calling Steve Opritza this morning and explaining what had happened.  Intermediate string will need to be cemented to surfaceOperator had also failed to notify when location was begun to be built and when rig moved in.  I told them they must test BOP and that even though surface casing was new, they needed to do test for integrity of cement and surface casing before drilling further.  I will continue to monitor the rig closely.

| This is not a production well at this time, as it has not been completed. Upon inspection, it was observed that the well location is stabilized with a vegetative cover. There are no tanks associated with this well. There is no production equipment, either. No ID is posted on the location. I will contact Flat Rock Development LLC and instruct them to post ID. No other violations are present that would prevent Temporary Inactive Status.

| Upon inspection, it was observed that a sparse vegetative cover has been established on the majority of the disturbed area, the lease road has been maintained, and the pad area is stabilized with gravel. The southeast slope of the location is developing a slip and is causing erosion. I will contact Mr. Scott Gannon of Flat Rock Development LLC and notify him of the pad erosion issues and recommend that the area be re-seeded and fertilized to encourage better growth. The preliminary restoration cannot be passed until the entirety of the location is stabilized.

| I received information that the location had been built and upon inspection, found this to be true.  Neither Carl Heinrich, partner, nor Flat Rock seemed aware that they were to give 24 hr. notice before beginning construction.  Location appears large enough but the south half built on fill - have a compactor on location and used it on the fill portion, they say.  They are now starting to bring in rock and pad will be covered with rock.  Road steep getting up to the location.  Called Flat Rock and informed them of all the situations that they needed to give 24 hr notice and told them to read their permit.  They are new as operators, both in Ohio and WVA.  WACO will be the drill company and rig should be in this week.  Asked them to call.

| Called for BOP, cement integrity on surface casing but when arrived, found they were having trouble with BOP - on location about 3 hrs.  Told to come back later at night, arrived to find testing crew there but BOP would not test - went for more parts - 5.5 hrs. this time.  Finally did test on 10/4 but I was out of hours - overtime denied (Supervisor Chini had scheduled meeting on 10/5/12 for I/O and had to save remaining hours for it - going to this BOP test would have put me on OV for 10/5).  Full Circle completed test with a graph and surface/BOP passed test.  Will go back to drilling.  10/6 - Rig called Sat. early to say Intermediate will be cemented this day.  Cannot witness as out of hours - no OV allowed.  During call, operator stated they would be going to 10,500'.  I told them they could only go to 8500', per the permit and will need to file a correction to the permit before they can go deeper.  Steve is concerned they may be at 8500' before Tues. morning - 1st time they can get approval - and I told them they could not drill deeper than 8500'.  I advised to complete amended form, fax to Columbus and call Steve Opritz for approval 1st thing Tues. morning.

| Upon inspection, it was observed that the preliminary restoration activities for this well have been completed. The containment berm around the perimeter of the well pad has been re-seeded/mulched and vegetation is growing in sufficiently enough to prevent significant erosion. This fulfills the requirements of the compliance notice.

1353258626 2012-10-26 My inspection this day due to plugging back of deeper formations in this well in anticipation of future horizontal well in Marcellus. Set bottom (Utica), Clinton/Medina, and Big Lime plugs and also a kick-off plug at Marcellus using Nabors Well Service and Class A cement. Total time for plugging was 18.5.
-1731302331 2012-10-24 Met with Cindy Vandyke on well that was drilled and was being plugged back with a coil tubing unit. The equipment was not able to make it in to location. Nabors was cementing company. They were on site and Waco rig was getting ready to run pipe in to set plugs.
-324912882 2012-10-17 Steve Bumgardner, consultant, called to say rig TD'd this morning at 7:30 at 10,220. Will start logging well tonight, then go back in and do side-wall coring. They may also do some formation testing if warranted. If not, will start plugging back on Friday or Sat.
-478613128 2012-10-03 My arrival found rig going in hole with 13 3/8 casing. I was unable to witness conductor cementing but had been to rig before drilling started and felt they had read their permit and pointed out to them when inspectors were to be notified. After the cement crew had cementing head on and trucks hooked up, ready to pump cement, I realized they had not met permit conditions which called for 500' of surface casing - they had only run 336' after drilling 17.5 hole to 361. They also did not set enough conductor, setting only 30' when the permit called for 100'. Again stressed the importance of reading their permit and let them go ahead and cement this lesser amount of surface casing, calling Steve Opritza this morning and explaining what had happened. Intermediate string will need to be cemented to surface. Operator had also failed to notify when location was begun to be built and when rig moved in. I told them they must test BOP and that even though surface casing was new, they needed to do test for integrity of cement and surface casing before drilling further. I will continue to monitor the rig closely.
617804020 2014-10-30 Upon inspection, it was observed that the preliminary restoration activities for this well have been completed. The containment berm around the perimeter of the well pad has been re-seeded/mulched and vegetation is growing in sufficiently enough to prevent significant erosion. This fulfills the requirements of the compliance notice.
