Per Dept records, well permitted by Mid East on 1-22-04, sold to North East Gas on 1-22-07. According to Dept records, well transferred to current Opr Kelly Hite on 1-7-13. doesn't appear prod reports have been submitted as required, nor has an update been received on inactive status.Dept rec'ds Opr recind inactive status of well to reflect current status, active.On 4-13(1535).Dept contacted Opr wife.Dept inquired on current status of well.Mrs. Hite stated well being used for house gas, stated Mr. Hite (Opr) would call Dept back.At 1545 same day, Mr. Hite stated he recv'd letter from Dept on reporting prod data but misplaced it, would like add'l guidance for reporting prod data.Viols being issued for illegible well tag & failure to report prod data.

NOV’s issued on 4-3-15 for failure to install permit number on well and failure to submit production data. Department left phone message for Mr. Hite on 5-22-15 concerning above violations. Mr. Hite returned call the same day. He stated well tag has been installed on well head. He attempted to submit production data but was unable to. Department gave him the phone number for the GreenPort help desk. Mr. Hite stated he would call Department “next week” to let me know if he was able to submit production data. A follow-up inspection will be conducted at a later date to ensure well head is properly identified.

As per Oil and Gas Act, permit number must be placed on well head in a ‘legible, visible and permanent’ manner. Another follow-up inspection will be conducted to ensure compliance. Outstanding violation still exists for failure to report production data as per current regulations. Inspected with WQS S. Foulk.

Department agreed to provide guidance documents by e-mail to Operator. A follow-up inspection will be conducted to verify well is properly identified with a well tag. Previously issued violations for this site are outstanding.

No brine tank on site. Site, located in yard near house, appears permanently stabilized. Department spoke with Robert Hite (Operator’s father) at site and agreed to send, via e-mail, information on PA GreenPort and the GreenPort help desk telephone number because violations outstanding for this site for failure to report production data.

Operator contacted Department by phone on 7-14-15 to discuss production data reporting on PA GreenPort. Stated the Operator was having trouble entering production data; that the website would ‘kick him out’ before data could be entered. Department recommended Operator call PA GreenPort help desk phone number listed on website. Department also provided Operator contact information for help with changing status of the well. Informed Operator that Department records show well as ‘inactive.’ If well is producing then well status shall be udpated to reflect 'active' status.

Depart contacted Operator Kelly Hite by phone on 7-23-15 to obtain update on reporting production data. Opr indicated production data still not entered. Depart recommended Opr use form sent with previous inspection to change status of well from ‘Regulatory Inactive’ to ‘Active’ thus updating Depart records. This may alleviate problems encountered with entering production data.UPDATE: Robert Hite contacted Depart on 7-24-15, left message, Depart returned message later that morning. Depart reiterated importance of changing well status to ‘active.’ Opr waiting for hard copy of form to allow him to report production data by mail.Depart contacted Robert Hite by phone on 7-24-15. According to Depart records, well status has been updated to 'Active' as of 7-24-15. Depart verified that production data must be entered electronically via PA GreenPort, as per Chapter 78.121, and Opr stated he would try PA GreenPort again. Violation still exists for failure to report production data.

Department spoke with Operator by phone on 8-11-15 and discussed well production reporting. Operator still having trouble inputting data into GreenPort. Department gave Operator GreenPort technical assistance phone number. Operator requested a ‘hard copy form’ be sent so he could mail production data to Department. Department stated this was not an acceptable method of reporting but would get back to Operator with additional info. Enclosed with this report is a copy of Oil and Gas Production and Waste Reporting Manual for use by Operator.
