These frac tanks are currently not located in secondary containment nor to they appear to be connected to anything. There are (6) 400 BBL produced fluids tanks located in modular secondary containment. Water within the containment is frozen and snow covered. In addition, there's a row of 22 frac tanks stationed along the E edge of the well pad. This series of 22 frac tanks are stationed in secondary man made containment with a berm. There are 8 gas production units stationed along the W edge of the well pad.

| I arrived onsite and met with Jeff Passarello of Seneca in response to a spill report.  Between 3 and 4 AM on 5/24, a worker on the site set up to pump produced water from both the Baker tank battery and the production tanks to the Clermont storage facility.  The same pump is used to pump water from the well cellars (when needed) along with the produced water, but no pumping was occurring at the cellars.  A water line is left in place to the cellars, but capped ~20' from the cellars to allow vehicle traffic through.  When the worker turned on the pump, the cap popped off of the water line to the cellars and spilled ~1 1/2 barrels of produced water from the pump to the pad surface.  When I arrived, approximately a third to a half of the spill area had already been excavated and a 30 yard dumpster filled.  Jeff informed me that they were waiting on another rolloff to continue the excavation.  The gravel was underlain by a clay layer, making visual confirmation of the spill removal possible, but Jeff was also u

There are a total of 8 drilled and producing wells located on this well pad. All well heads are properly tagged. Well cellars are full of water. No bubbling or sheen observed in the cellars. There are (6) 400 bbl. produced fluids tanks stationed in secondary containment. Approximately 1" of rain water was found in the produced fluids secondary containment with a conductivity reading of 78.8 uS/cm. A series of 12 manifolded frac tanks are being stationed on secondary along the NE edge of the well pad. Just a trace of water was observed within this containment. Overall, the well site looks good. No violations noted.

Rig mats remain onsite with a total of 25 produced fluids frac containers. It appears that the operator is gearing up for well stimulation. All wells are labeled with the well number and operator. A total of 9 wells have been TD. Well 023-20182 has been drilled and plugged.

There are 6 produced fluids silos located in secondary containment onsite. Field screen reading for specific conductance within this containment was 82.0 uS/cm.

Overall, the pad is in good condition and all necessary BMPs are in place surrounding the well pad.

The sign located at the entrance to the well pad only has a total of 7 well listed. This does not match with the number of drilled wells onsite. I recommend updating the well site to reflect the number of drilled wells onsite.

2627524 Primary Facility 2017-08-18 Routine/Complete Inspection No Violations Noted Routine inspection of Seneca Resources Corp CRV PAD C09-G on August 17, 2017 with company man Adam Pierce of Seneca Resources.

Crews were currently working on fracing Marcellus wells 023-20178, 023-20179, 023-20183, and 023-20184. The fracing crew was approximately half way done with fracing these four wells. Once finished they will move the rig over and work on stimulating Marcellus wells 023-20180, 023-20181, 023-20182, and 023-20198. According to Mr. Pierce, crews are installing a perforation string downhole and with 8 perforation holes per every 2 ft. section of the string. Plugs made of a composite are being placed between 190 ft. perforation stages.

Sand is being hauled via trucks from the Cameron County rail yard. Water used for stimulation is being transferred to this pad via the above ground water line from the Clermont Water Treatment Facility. The above ground waterline is plumbed into a series of manifolded frac tanks.

2643030 Primary Facility 2017-10-03 Routine/Complete Inspection No Violations Noted Routine inspection of Seneca Resources Corp CRV PAD C09-G on September 28, 2017 with company man Scott Digel of Seneca Resources.

Crews were currently working on fracing Marcellus wells 023-20182, 023-20183, and 023-20184. The fracing crew was almost completed with well stimulation on these 3 wells. Stimulation been completed on wells 023-20178, 023-20179, 023-29180, 023-20181, and 023-20198. According to Mr. Digel, once all of the wells have been stimulated, crews will remove this rig and bring in 2 work-over rigs to drill out the plugs on the above mentioned wells.

The tubing pressure(production) was 1064 psi. at this inspection. Checked all fittings and vented annulus 20x13, 13x9, and 9x5 at this inspection. Pad and well area were neat and organized.
2672481 Primary Facility 2017-12-15 Routine/Complete Inspection No Violations Noted Routine inspection of Seneca Resources CRV PAD C09-G on December 15, 2017.

There are a total of 8 producing Marcellus wells located on this pad. Rig mats from the recent flowback operation have been removed and are currently being stationed along the N edge of the well pad. There is one frac tank stationed on man-made secondary containment with a berm near the well heads. There are 4 frac tanks located near the produced fluids containment along the S border. These frac tanks are currently not located in secondary containment nor to they appear to be connected to anything. There are (6) 400 BBL produced fluids tanks located in modular secondary containment. Water within the containment is frozen and snow covered. In addition, there's a row of 22 frac tanks stationed along the E edge of the well pad. This series of 22 frac tanks are stationed in secondary man made containment with a berm. There are 8 gas production units stationed along the W edge of the well pad.

2707931 Primary Facility 2018-03-20 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. The wellhead assembly, annulus vents 20" x 13 3/8", 13 3/8" x 9 5/8" and 9 5/8" x 5 1/2" were tested for gas. Results of this testing was that 100% LEL(lower explosion limit) and 42% Methane was detected on the 9 5/8" x 5 1/2" annulus vent. No other gas was detected on the wellhead assembly or the well area.
2734716 Primary Facility 2018-05-24 Incident- Response to Accident or Event No Violations Noted I arrived onsite and met with Jeff Passarello of Seneca in response to a spill report. Between 3 and 4 AM on 5/24, a worker on the site set up to pump produced water from both the Baker tank battery and the production tanks to the Clermont storage facility. The same pump is used to pump water from the well cellars (when needed) along with the produced water, but no pumping was occurring at the cellars. A water line is left in place to the cellars, but capped ~20' from the cellars to allow vehicle traffic through. When the worker turned on the pump, the cap popped off of the water line to the cellars and spilled ~1 1/2 barrels of produced water from the pump to the pad surface. When I arrived, approximately a third to a half of the spill area had already been excavated and a 30 yard dumpster filled. Jeff informed me that they were waiting on another rolloff to continue the excavation. 2792977 Primary Facility 2018-10-16 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Seneca Resources CRV C09-G Well Pad on October 16, 2018. There are a total of 8 drilled and producing wells located on this well pad. All well heads are properly tagged. Well cellars are full of water. No bubbling or sheen observed in the cellars. There are (6) 400 bbl. produced fluids tanks stationed in secondary containment. Approximately 1" of rain water was found in the produced fluids secondary containment with a conductivity reading of 78.8 uS/cm. A series of 12 manifolded frac tanks are being stationed on secondary along the NE edge of the well pad. Just a trace of water was observed within this containment. Overall, the well site looks good. No violations noted.

2808928 Primary Facility 2018-11-29 Routine/Complete Inspection No Violations Noted Seneca Resources submitted a spud notice to the Department via the Greenport system on November 21, 2018 for the following Utica wells: 023-20238 through 023-20243.

Department representative Sue Herzing conducted a routine site inspection at Seneca Resources CRV Pad C09-G on November 29, 2018. Each 20" round 100' in length conductor was in-place for the above listed wells. In addition, there are a total of 8 producing Marcellus wells located on the pad.

Overall, the well pad was snow covered and frozen. No signs of leaks or spills were observed on the well pad.

2843517 Primary Facility 2019-02-21 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was not in production (temporarily shut in). 915 psi (pounds per square inch) was showing on the production/ flow line at the time of this inspection. This well has 3 annulus vents vented to surface. The annulus vents were all tested for combustible gas with a Altair 5 x Multi gas meter. The results were the 20" x 13" tested 0% LEL (lower explosion limit) and 0% METHANE. 13" x 9" tested 0% LEL (lower explosion limit) and 0% METHANE. The 9" x 5" tested 100% LEL (lower explosion limit) and 41% METHANE. This meter was calibrated on 2-20-19. No violations noted at this inspection.
