2586492 Primary Facility 2017-04-21 Administrative/File Review Outstanding Violations - Viols Req'd At 1521, the Department contacted Operator representative Mark Gawel in response to a voicemail left for the Department by Mr. Gawel.

Mr. Gawel called to seek guidance on how to reach compliance and correct violations previously issued under inspection record 2574463 (enf ID 351894).

The Department explained that an E&S plan is to be available at the well site during all stages of an earth disturbance activity and that E&S BMPs shall be installed in accordance with this E&S plan.

Mr. Gawel stated that an E&S plan will be made available at the well site and temporary E&S BMPs will be installed as appropriate. Mr. Gawel will keep the Department updated.

Violations remain outstanding at this time.

2588880 Primary Facility 2017-04-25 Administrative/File Review In Compliance with Policy At 0932, operator representative Mark Gawel submitted the plugging E&S plan for this site to the Department via e-mail.
2591656 Primary Facility 2017-05-08 Compliance Evaluation No Violations Noted On-site CEI conducted in response to violations previously issued under inspection record 2574463 (enf ID 351894) for failure to have an E&S plan on site for all stages of an earth disturbance activity and for failure to implement E&S BMPs to minimize the potential for accelerated erosion and sedimentation.

At time of inspection, E&S plan on-site. Temporary E&S BMPs installed per the E&S plan. Site temporarily stabilized with mulch. No accelerated erosion and sedimentation issues observed at this time.

Utilizing a range-finder, it appears that an approximate area of 23,500 square feet was previously disturbed. This area does not include any earth disturbance thar may have occurred on the access road.

Violations previously issued under inspection record 2574463 (enf ID 351894) have been corrected. No new violations noted at this time.
