| Flowback procedures at the site have been completed relatively recently.  Carrizo was in the process of breaking down equipment at the site.  Residual waste vacuum trucks were in the process of unloading flowback fluid from frac tanks in the tank battery.  Mr. Bonnice stated that the 4H well is not in the production phase of operations.  The well has been flowed back.  Aboveground flow lines have been installed as a temporary measure to route the gas and fluid from the 1H-3H wells to gas production units and production tanks.  Eventually, underground flowlines will be installed at a depth of (4) feet to route the gas and fluid from the wells to the gas production units and production tanks.  I observed tears in the liner where the gas buster tanks are stored.  Mr. Bonnice and Mr. Sodon informed me the gas buster tank containment system would be repaired as soon as possible.

Carrizo was in the process of breaking down equipment at the site. Residual waste vacuum trucks were in the process of unloading flowback fluid from frac tanks in the tank battery. Mr. Bonnice stated that the 4H well is not in the production phase of operations. The well has been flowed back. Aboveground flow lines have been installed as a temporary measure to route the gas and fluid from the 1H-3H wells to gas production units and production tanks. Eventually, underground flowlines will be installed at a depth of (4) feet to route the gas and fluid from the wells to the gas production units and production tanks. I observed tears in the liner where the gas buster tanks are stored. Mr. Bonnice and Mr. Sodon informed me the gas buster tank containment system would be repaired as soon as possible.

The remainder of the site was stable and well vegetated with no apparent erosional issues. The last company inspection of the erosion and sedimentation controls of the site was on 3/7/2016.

There were no violations.

The well site itself is in good shape, with no erosional issues and all vegetation was at or exceeding the 70% vegetative threshold for perennial growth. All BMPs had been removed, and the site was stable.

I recommend APPROVAL of this NOT.

2602081 Primary Facility 2017-06-07 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:00a.m.

The cellar had water at the bottom with no bubbling noticed. The valve on the (13x9) annulus was closed. The (9x5) annulus vent pipe was open and venting into the production tank. There was 594psi on the production casing and 319psi on the production line. I took a walk around the location and there were no obvious concerns noticed. There was a 500bbl flow back tank on containment and the containment appeared to be in good condition. I left the site at 11:40a.m.

2688979 Primary Facility 2018-01-30 Routine/Complete Inspection No Violations Noted On January 30, 2017 I inspected the Trekoske well pads. The access road was very solid and stable. The cut and fill slopes were well vegetated where not stone lined. The wells were all clearly and properly labeled and the pad surface was solid. The surrounding farm field edges had been previously undergoing enhancements and cleanup by the landowner near the access road, but the work has been completed and is now growing well and stable.
