2432904 Primary Facility 12/08/2015 Drilling/Alteration No Violations Noted Alpha Hunter Drlg Rig #6 pulled junk basket. Will make up BHA, RIH, 1000' to drill to TD. Packer job likely Thursday PM. NOVIO
2432908 Primary Facility 12/10/2015 Drilling/Alteration No Violations Noted Alpha Hunter Drlg Rig POOH DP, RIH with csg in 2-3 hours. Likely ready for packer job 3-4 PM. NOVIO
2435208 Primary Facility 12/22/2015 Drilling/Alteration No Violations Noted Alpha Hunter Drilling Rig #6
2441680 Primary Facility 12/10/2015 Drilling/Alteration No Violations Noted Alpha Hunter 6 Rig. Well # 593962. On site to observe the installation of the casing annulus packer (CAP) on the 9 5/8” casing string. The CAP was installed at the predetermined formation depth (1,791’-1,803’). Borehole was conditioned. The 9 5/8” casing was cemented with 42 bbls cement displaced to surface. Bumped plug, floats held. Higher than normal pump pressures were observed throughout the job. After landing the plug the pressure was increased to ensure CAP inflation. Pressure was lowered to activate the locking system on the CAP. A bond log will be performed to evaluate the cement job and verify that the CAP performed as designed.
2444527 Primary Facility 2016-01-26 Drilling/Alteration No Violations Noted Alpha Hunter Drilling Rig #6

Reentered this well on 1/20/16 drilled 8 ½” wellbore to 5296 ft.

2473711 Primary Facility 2016-04-01 Drilling/Alteration No Violations Noted Orion Drilling Rig #17

Moved onto this well on 3/21/16 and resumed drilling at 5296 ft. KB. KOP was at 5360 ft. EOB on the curve was at 9229 ft. MD (8056 ft. TVD). TD was at 18,169 ft. MD (7918 ft. TVD). on 3/29/16. The 5 ½” casing was set at 18,169 ft. The estimated TOC is at 3663 ft.

2720530 Primary Facility 2018-04-18 Routine/Complete Inspection No Violations Noted This well has been completed and is currently producing. Reclamation of the pad is underway. The pad and equipment appear to be well maintained.
2735892 Primary Facility 2018-05-18 Routine/Complete Inspection No Violations Noted On Friday, May 18, 2018, I conducted a routine-complete inspection of the Koci Well Site located in West Bethlehem Township, Washington County.

Gas well 593962 and the associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

No violations were identified during the inspection.
