| I met Dan Denison the landowners brother. The old well is on the Plug and Abandoned status but the 8''casing is open with a tree root growing a crossed it. I dropped a rock and the depth is deep but unknown. The well was a Standard rig drilled hole and the all pillars are in place. No gas or oil leaking at present time. Landowner named Mike Dennison is going to build a house where old well is.

| An inspection conducted with Mineral Resources Inspector 3, Mr. Pat Shreve, revealed a well that consisted of only open 9.38 diameter drive pipe with a fluid level at approx 400' and no leakage of natural gas smelled or detected with a meter at the surface.  A tree with approx 1' diameter trunk was observed next to the wellbore and would be an overhead obstruction for a rig.  The well site contained multiple concrete structures that were from a standard rig and their measurements were as follows: Four pedestals each measured approx 4' x 4' x 2'; Power Arm at 12.5' x 6'; and a concrete pad at 12.75' x 9'.  The well was situated within a remote wooded area that did not contain a nearby residential dwelling or water well within 300'.  A GPS reading was obtained for a potential access point into the well location.  A review of the RBDMS well card indicated that well had been plugged and abandoned.

| A follow-up inspection revealed that the well status had remained unchanged since the last inspection.  The well consisted of only one visible string of casing at approximately 2 below the surface with a tree root grown across the middle. Casing calipers were placed around the outside of this casing to show 9.25 diameter.  A depth counter equipped with a braided line and a 2 oz sinker was utilized for this inspection to gauge the wellbore depth of the open 9.25 outside diameter casing.  The total depth was measured at 126' and the material that was encountered resemebled a soft clay-like material.  At approximately 50', the wellbore had appeared to contain a ledge with a smaller diameter wellbore below.  During the retrieval of equipment from the wellbore, a visual inspection of the line and sinker showed no trace of crude oil.  A natural gas odor was also not smelled or detected with a gas meter.  For access point into the location refer to the above GPS reading and also look for mailbox with address 10140 Sidle Rd.

| On the day of my inspection I visited the site with Muskingham County Inspector Pat Shreve and Orphan Well Inspector Brent Bear. We measured the depth of the well to 184' with a weighted line. The only visible casing was callipered at 9.625. There was no gas detected and no signs of leaking fluids.

| An inspection of this well was conducted with Pat Shreve and Chris Grimm to verify the previously noted total depth of 126' with a heavier weight.  This inspection revealed a deeper total depth of 184'.  During the retrieval, it was noted that both the braided line and weight did not contain crude oil.  The wellbore was monitored with a gas meter and showed no detection of natural gas.  This casing was visible but situated approximately 2 below ground with a tree root across the middle.  After the removal of this material with a shovel, the casing measured 9.63 (outside diameter) with a caliper.

| Brent Bear, Chris Grimm and I looked at well casing and entrance point. We checked depth with line counter and plumb bob. The depth was at 184'. There could be cast ball and lead wool setting in well bore. The size of the casing is 9.625'' after taking shovel and digging around. This has been placed in the program to plug. The landowner will be building house on hill top in September.

1359038047 2013-01-24 Orphan No Violations I met Dan Denison the landowners brother. The old well is on the Plug and Abandoned status but the 8casing is open with a tree root growing a crossed it. I dropped a rock and the depth is deep but unknown. The well was a Standard rig drilled hole and the all pillars are in place. No gas or oil leaking at present time. Landowner named Mike Dennison is going to build a house where old well is.
1364465325 2013-03-26 Orphan No Violations I met Brent Bear and we went in to location and took pictures and checked depth of well. We established an entrance. This is a complaint from Mike Dennison. Complaint #is 4202.
1366375643 2013-03-26 Orphan No Violations An inspection conducted with Mineral Resources Inspector 3, Mr. Pat Shreve, revealed a well that consisted of only open 9.38 diameter drive pipe with a fluid level at approx 400 and no leakage of natural gas smelled or detected with a meter at the surface. A tree with approx 1 diameter trunk was observed next to the wellbore and would be an overhead obstruction for a rig. The well site contained multiple concrete structures that were from a standard rig and their measurements were as follows: Four pedestals each measured approx 4 x 4 x 2; Power Arm at 12.5 x 6; and a concrete pad at 12.75 x 9. The well was situated within a remote wooded area that did not contain a nearby residential dwelling or water well within 300. A GPS reading was obtained for a potential access point into the well location. A review of the RBDMS well card indicated that well had been plugged and abandoned.
1366984779 2013-04-26 Orphan No Violations A follow-up inspection revealed that the well status had remained unchanged since the last inspection. The total depth was measured at 126 and the material that was encountered resemebled a soft clay-like material. At approximately 50, the wellbore had appeared to contain a ledge with a smaller diameter wellbore below.
1391457471 2014-02-03 Orphan No Violations On the day of my inspection I visited the site with Muskingham County Inspector Pat Shreve and Orphan Well Inspector Brent Bear. We measured the depth of the well to 184 with a weighted line. The only visible casing was callipered at 9.625. There was no gas detected and no signs of leaking fluids.
1392227793 2014-02-03 Orphan No Violations An inspection of this well was conducted with Pat Shreve and Chris Grimm to verify the previously noted total depth of 126 with a heavier weight. This inspection revealed a deeper total depth of 184.

There could be cast ball and lead wool setting in well bore. The size of the casing is 9.625 after taking shovel and digging around. This has been placed in the program to plug.
