| I did a Compliance Notice and Complaint # 4933 follow-up inspection for this well.  The well is plugged.  I called the complainants and left a message for them to call me back concerning the closing of complaint #4933.  The well is plugged and the Notice of Compliance issued 02-28-2017 is passed..

| I was on location with Stark County Inspector Shawn Murphy to witness the level of the surface cement plug, cutting off of the casing below grade and attachment of the ODNR I. D. plate. Nicks Well Plugging cut off the drive pipe and surface casing approximately five (5) feet below grade. The cement plug was present at this level and set. No indication of gas migration was observed. The six (6) inch casing for the associated water well was cut off at this level and instructions were given to plug it with sacked cement. No violations identified during this inspection.

| I was on location to assist Shawn Murphy with the plugging of the Engleman # 1. Previously, the 5.5 casing was parted at 3050 by Appalachian Well Surveys and Nicks Well Plugging (NWP) removed 31 joints of badly corroded 5.5 casing. The corrosion included both pitted areas and holes in the casing. A work string of 1.5 tubing was lowered into the well with the bottom located at 1007 ( 31 joints). NWP circulated a cement slurry through the interval 1007 to 807. Shawn Murphy verified the cement slurry weight by scale. NWP removed 25 joints to approximately 200 and circulated a cement slurry back to surface. After the displacement of the slurry ceased the cement top was located at five (5) feet down the 8.25 surface casing and was found there 40 minutes after the job.

| I conducted a complaint follow-up inspection on this well.  Complaint #4933 was received on 2-10-2015 and Inspector William (Fred) Romeo conducted the initial inspection.  The well is an open hole completion and has a 50 barrel tank.  The well is not legibly identified.  The well is owned by Don Geitgey this was affirmed research done by Fred Romeo and Heidi Scott (Oil & Gas Inspector).  In November of 2015 an ownership transfer packet was sent to Don Geitgey for this well.  Well transfers for 151-20321 & 151-20314 were also included in the packet.  Three change of ownership packets with form 7s were received 11-18-2016 by the Columbus office of Oil & Gas.  Legal did not approve the Historic owner transfer.  Three new ownership forms were sent to Don Geitgey on 01-11-2016 and have not been returned - Reference E-mails Wendy Booth to and from Kathryn Smith dated 06-09-2016.  I contacted Don Geitgey on 11-01-2016 he stated he had received the transfer forms but did not understand how to fill them out.

| I conducted a complaint follow-up inspection on this well for complaint #4933.  I spoke with the complainants and they stated the well has been idle since they have owned the property.  This a swab well and the well is shut-in and no annual production reports have been submitted since 1987.  The well is not legibly identified.  There are no flowlines or sale lines connected to the well.  There is a domestic gas connection to the southwest of the well which is not in use and disconnected.  This well presently is incapable of production.  Consequently, this well shall be plugged in accordance with Ohio Revised Code section 1509.12(B).  The well owner/operator shall submit a written plugging schedule to Shawn Murphy at 3575 Forest Lake Dr., Ste. 150, Uniontown, Ohio 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days.  The well owner/operator will be notified if the plugging schedule is approved or denied.

| I am on location with Bob Roush (MRI3) to witness the cutting of the casing and plating of the well which is associated with complaint #4933.  The cement is at surface.  The surface casing was cut a minimum 30 inches below ground level and plated with an identification.  The water well casing (used for drilling 20322) was cut below grade and topped off with sakrete.  I spoke with complainants/ landowners Mrs. Sampsell on 10/19/2017 and Mr. Sampsell on 10/20/2017.  Both are in agreement about closing complaint #4933.  I updated the complaint log and recommend the complaint be closed.

0228651409 2017-10-20 Plug / Plug Back No Violations I was on location with Stark County Inspector Shawn Murphy to witness the level of the surface cement plug, cutting off of the casing below grade and attachment of the ODNR I. D. plate. Nicks Well Plugging cut off the drive pipe and surface casing approximately five (5) feet below grade. The cement plug was present at this level and set. No indication of gas migration was observed. The six (6) inch casing for the associated water well was cut off at this level and instructions were given to plug it with sacked cement. No violations identified during this inspection.
0711844203 2017-10-19 Plug / Plug Back No Violations I was on location to assist Shawn Murphy with the plugging of the Engleman # 1. Previously, the 5.5 casing was parted at 3050 by Appalachian Well Surveys and Nicks Well Plugging (NWP) removed 31 joints of badly corroded 5.5 casing. The corrosion included both pitted areas and holes in the casing. A work string of 1.5 tubing was lowered into the well with the bottom located at 1007 ( 31 joints). NWP circulated a cement slurry through the interval 1007 to 807. Shawn Murphy verified the cement slurry weight by scale. NWP removed 25 joints to approximately 200 and circulated a cement slurry back to surface. After the displacement of the slurry ceased the cement top was located at five (5) feet down the 8.25 surface casing and was found there 40 minutes after the job.
1411898472 2017-11-06 Plug / Plug Back No Violations I did a status check on the well site. The site has been graded, seeded and mulched. Vegetation is starting to establish. A final reclamation will be done at a later date. A landowner waiver for the inground flowlines has not been received at this time.
1478000589 2016-10-31 Production Wells No Violations I conducted a complaint follow-up inspection on this well. Complaint #4933 was received on 2-10-2015 and Inspector William (Fred) Romeo conducted the initial inspection. The well is an open hole completion and has a 50 barrel tank. The well is not legibly identified. The well is owned by Don Geitgey this was affirmed research done by Fred Romeo and Heidi Scott (Oil & Gas Inspector). In November of 2015 an ownership transfer packet was sent to Don Geitgey for this well. Well transfers for 151-20321 & 151-20314 were also included in the packet. Three change of ownership packets with form 7s were received 11-18-2016 by the Columbus office of Oil & Gas. Legal did not approve the Historic owner transfer. Three new ownership forms were sent to Don Geitgey on 01-11-2016 and have not been returned - Reference E-mails Wendy Booth to and from Kathryn Smith dated 06-09-2016. I contacted Don Geitgey on 11-01-2016 he stated he had received the transfer forms but did not understand how to fill them out.
1488228769 2017-02-24 Production Wells Violations Noted I conducted a complaint follow-up inspection on this well for complaint #4933. I spoke with the complainants and they stated the well has been idle since they have owned the property. This a swab well and the well is shut-in and no annual production reports have been submitted since 1987. The well is not legibly identified. There are no flowlines or sale lines connected to the well. There is a domestic gas connection to the southwest of the well which is not in use and disconnected. This well presently is incapable of production. Consequently, this well shall be plugged in accordance with Ohio Revised Code section 1509.12(B). The well owner/operator shall submit a written plugging schedule to Shawn Murphy at 3575 Forest Lake Dr., Ste. 150, Uniontown, Ohio 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

This is a swab well with a swab tank. The well is shut in and there is a domestic gas meter covered in vegetation ~ 30 ft. west of the well. I met with Don Geitgey Jr. (Owner) and Nick (Nicks Well Plugging) on location. The well is scheduled to be plugged the week of 10-09-2017. I notified the complainant and updated the complaint log.

Nicks Well Plugging is the plugging and cement contractor for the plug job. The bottom and top surface plugs were placed through tubing. Circulation was established prior to placing the plug.
1508509584 2017-10-20 Plug / Plug Back No Violations I am on location with Bob Roush (MRI3) to witness the cutting of the casing and plating of the well which is associated with complaint #4933. The cement is at surface. The surface casing was cut a minimum 30 inches below ground level and plated with an identification. The water well casing (used for drilling 20322) was cut below grade and topped off with sakrete. I spoke with complainants/ landowners Mrs. Sampsell on 10/19/2017 and Mr. Sampsell on 10/20/2017. Both are in agreement about closing complaint #4933. I updated the complaint log and recommend the complaint be closed.
