| On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units, (4) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. This well is setup to be produced by means of a plunger lift system. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. It will be noted that in 2018 a slip had occurred along the southern slope of the pad and western slope of the lease road, prior to the pad. The slopes have since been graded, seeded, and erosion control in place. Currently awaiting new vegetation.

|  Status check inspection found the well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 0 pounds pressure and the 9.625 casing well head reads 0 pounds pressure. On pad total is (8) complete wells supported by (8) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.

Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0460192368 2014-04-03 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Patterson rig 332 on the hole and has drilled 15 out from under the recently set and cemented 13.375 2nd intermediate well bore section and has conducted a blow out preventer and formation integrity tests.
0933052089 2014-04-14 Drill / Deepen / Reopen No Violations Status check inspection of the drilling progress found Patterson drilling rig 332 on location drilling the 8.5 diameter production bore hole at a current depth of 10,600 in the horizontal well bore section with a current directional azimuth of 329 degrees, the horizontal curve kick off point was at 7,409 and the landing point was at 9,917.
1030844537 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance.
1143191059 2014-10-01 Completion Testing No Violations Status check inspection of the completion process found the well stimulation process complete and key Energys workover rig on location, the well bore process of drilling out the completion plugs to total depth has been completed and the rig is pulling out of the hole with the drill string and shall run the 2.375 production tubing in the hole.
1395654385 2014-03-24 Casing String Witnessed No Violations The said well was spudded on 03/23/2014 by Express Energy top hole rig, 100 of 26 conductor was cemented to surface on 03/24/2014 by Express Energy cement services with 330 sacks of class A cement ran and a 50% calculated open hole cement excess ran with 15 barrels of cement returns to surface.
1395949547 2014-03-27 Casing String Witnessed No Violations On 03/27/14 472 of 18.625 surface casing was ran and cemented to surface by Allied cement services using 931 sacks of class A- 2% calcium .125 pound per sack flow seal.
1396356705 2014-04-01 Casing String Witnessed No Violations Status check inspection found the 1st intermediate 17.5 well bore total drilling depth has been reached at 2,274 with a 13.375 54.50# casing string ran with a guide shoe and float collar used along with proper centralizer placement demonstrated.

1502621594 2018-03-07 Production Wells No Violations Status check inspection found the well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position.
