| I am on site today to witness the running and the cementing of the 20 inch diameter casing string. Express Drilling drilled a 26 inch diameter hole to 120 ft. and set 120 ft. of 94 lb. casing. Express Energy Service cemented with 230 sacks of class A cement at 15.6 lb. and a yield of 1.20  they did not circulate cement to surface. It was discovered that the top of the cement was 15 ft. from surface and needs to be topped off.

| I met with Mackey Lambert Drilling Consultant for Nomac Drilling  Rig 77 and discussed the drilling and permit conditions. This location has the potential of having H2S ( Big Lime). At the entrance to the location the drilling permit and conditions are posted and  warning signs with  three (3) wind directions socks thru out the drilling pad. There is monitors in the cellar and drilling floor and at the gas buster. There is an alarm in the Company trailer. All muster points are marked. At present there are no noted violations.

| I arrived today to check on the drilling status of Nomac Drilling Rig # 77 and found that they are a 8.5 inch diameter hole on oil base drilling mud at 14,000 ft. I checked on the drilling solids controls and liner stability and did not find any problems. It is very cold out today and the location is covered with ice and snow. Poor roads all around the area due to conditions

| I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked.  The permitted well is chained off with all valves in the closed position. RBDMS production records show zero production for the well. It appears that the gas line has yet to be connected to the well allowing for the production of gas.
 +
The production equipment on the pad consists of the following:
 +
6 400 bbl. Production tanks
 +
1 Well bore, that was shut in at the time of my inspection.
 +
1 Sand separator
 +
 +
1  Horizontal heater-treater
 +
1 Low pressure separator tower/Vapor recovery system unit.
 +
1 Combustor
 +
1 Gas compressor unit.

There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

| On the day of my inspection I found the Garvey pad to contain one properly identified wellhead and three cellars with conductor casings. Production equipment includes four gas production units with four vertical sand seperators. Six 400 barrel steel tanks with identification contained in a metal dike. Three are produced water and three are condensate.  Annualar pressure on the 9 5/8 inch diameter casing is 550 psi, 13 3/8 annular pressure is 0 psi. Well is equipped with a christmas tree style wellhead with chains on all valves. Well is loaded with water and a backpressure valve until drilling of the next three wells are completed. Production records are current in RBDMS. Lease road is gated and pad is fenced and locked.  Site passes preliminary restoration according to ORC 1509.072A.
