| After reviewing the cement bond log that was performed by Renegade Services on 12-30-2014 and conferring with manager Brent Bear the decision was made to require 1000' of separation between the cement top at 7241' and the upper most perforation.  At 1:40 PM, I called Steve Self, Completions Manager for Antero Resources, and discussed the 1000' requirement.  Mr. Self agreed to the condition that well not be perforated above 8241'.

| The purpose of my inspection was a status check on a production well. The well pad name is Cynthia and it consists of 5 wells. The well was equipped with a production tree style wellhead. Production valves were in the open position. Annular pressure monitoring was present. The gauge on the production casing read 280 pounds per square inch and the gauge on the production tubing read 170 pounds per square inch. The production equipment consisted of eight 400 barrel tanks within steel line containment and five gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and three combustors. The pipeline tie in was located on the North side of the pad. A dust suppressant had been recently applied to the lease road. The identification was located at the wellhead and the production records were current.

| After reviewing the cement bond log that was performed by Renegade Services on 12-30-2014 and conferring with manager Brent Bear the decision was made to require 1000 of separation between the cement top at 7241 and the upper most perforation.  At 1:40 PM, I called Steve Self, Completions Manager for Antero Resources, and discussed the 1000 requirement.  Mr. Self agreed to the condition that well not be perforated above 8241.
