| I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: The wellhead plunger lift and associated tank battery located on a larger residential parcel off of Fairmount Blvd.  The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery, and found no signs of impact or degradation attributable to the production system. The egress and ingress road was in good condition.

| On 01/04/2012, DNR Inspector Richard Skidmore completed an urban preliminary restoration well inspection for DNR Inspector Ray Wallace.  The off-set board-on-board fencing at the well head and tank battery are in good condition.  Proper identification signs were posted on both the tank battery and well head fences.  The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05.  Inspection passed.

| The producing unit consists of a mechanical plunger lift unit and above-ground storage tank (AST) battery. The battery is comprised of 1 100-barrel steel production tank and 1 50-barrel brine tank, and separator unit. The wellhead is not enclosed. The battery is enclosed in a board-on-board fence. The load-outs are bull-plugged. The ID and secondary containment are good. The unit is fitted with carbon canister, low-pressure relief valve, and spark arrestor. Gates are locked. Lease road is in good condition and areas impacted by drilling have been restored. No problems noted and passes preliminary restoration.

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0267798791 2017-10-10 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity.
0628902146 2012-01-04 UrbanPreliminary Restoration No Violations On 01/04/2012, DNR Inspector Richard Skidmore completed an urban preliminary restoration well inspection for DNR Inspector Ray Wallace. The off-set board-on-board fencing at the well head and tank battery are in good condition. Proper identification signs were posted on both the tank battery and well head fences. The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05. Inspection passed.
1382448319 2013-10-22 UrbanPreliminary Restoration No Violations The producing unit consists of a mechanical plunger lift unit and above-ground storage tank (AST) battery.
