1854244 2009-12-10 Outstanding Violations - No Viols Req'd Inspection performed due to the fact that the well is being plugged and the well is located in a potentially wet area. There is a small stream that splits and runs to either side of the well site. As was previously discussed with the operator, a culvert was installed on the access road to carry the stream underneath. Temporary Silt fence and hay bales were installed along the stream bed. At this time there is no evidence of erosion or sedimentation on the site or the access road. However, the operator should continue to monitor the site until restoration activities are complete.
1861969 2010-01-21 No Violations Noted plugged/vent with ID installed
1871199 2010-03-16 No Violations Noted inspection conducted with operator - verified monument
1984882 2011-01-05 No Violations Noted Monument Located. Site is restored and vegetation is well established. Rock Construction entrance has been removed.

NOTE- Operator can remove the silt fence after the spring thaw. In addition, the old cross-pipe is laying off to the side of the road. The operator should remove the cross-pipe from the site when they return to take out the silt fence.

There is some confusion as to which well was plugged. Some of the paperwork seems to have been mixed up between the 005-00683 well and the 005-00483 well.
