| I inspected this location at the request of the Columbus Office.  Apparently, no plugging report was ever submitted for this well but final restoration was approved on 12/14/1992.  A check of files in the Uniontown Office found a plugging inspection report from inspector M. Lee Mulvaney indicating that the well was plugged and abandoned on 12/05/1988 and the plug job was witnessed by the inspector.  The report indicated the depth of the plugs and the amount of casing recovered during the job.  Three photos of site restoration were also found and dated 06/05/89 by inspector Taylor.  I reviewed current and historical aerial imagey of the location and plotted the well on an aerial map using latitude and longitude coordinates from RBDMS.  It was apparent on the aerials and during inspection that the old roadbed used for access to the location was never removed.  There was no well or production equipment found during the inspection.  The roadbed was part of the yard and grass growing in the roadbed was not as healthy as the rest of the yard.  The roadbed intersected the driveway approximately 150' into the property which appears to be built across the old location of the surface equipment based on the outline at the east edge of the driveway.  Aerial photos from 1980 appear to confirm the presence of surface equipment at this location.  The wellhead location plots approximately 50' SSE from this point.  Visual inspection of this area did not find any indication of the plugged and abandoned well or of any impacted area that might indicate the plugs were leaking.  No violations.
