| I was on location to follow up on complaint #4291 filed on 4/23/13 about a pin hole leak in a production tank.  I met with the landowner, Dan Flick, and inspected the tank battery.  The battery contained a horizontal separator, a meter run, 1-210 barrel stock tank, and a 50 barrel poly tank.  The battery was diked and the proper ID was posted on the 210 barrel.  There was a small pin hole leak at a weld in the 210 barrel tank four feet off the ground.  Leakage was minimal and a small amount of oil had reached the ground inside the dike.  I called Kyle Ahbel (Enervest) and told him they need to fix the pin hole in the tank and clean up any oil on the ground.  He told me they would have a truck out tomorrow to empty the tank and they would get it fixed.  I inspected the wellhead and pumping unit.  The pumping unit was electric and was fenced and diked.  ID was posted on the fence.  No violations.

| I was on location at the request of the landowner to inspect the tank battery.  The 210 barrel tank had 2 pinhole leaks.  The first one, I had Enervest fix last year and is beginning to leak again.  The second, is on a welded seam below the first one and shows staining on the tank down to the ground.  An absorbent pad was on the ground at the tank base to soak up the leaking product.  Also, the connection to the poly tank is damaged and the gasket between the tank and the flange is broken in half and not in place.  The landowner showed me the drainage path from the tank battery area towards his house to the east.  An area of scattered, standing water, roughly 40' long through his yard had an oily sheen on top of it.  I confirmed it was oil with an absorbent pad and with my fingers.  Recent, heavy rain has mobilized residual oil inside the tank battery.  The inabiblity of the dike to contain spills and releases inside the battery is apparent based on the presence of sheen 150' down slope of the tanks and the lack of any standing water within the battery.  The height of the dike walls does not meet SPCC requirements and the southeast corner contains soil cracks that will allow the passage of liquids.  Enervest Operating LLC will, within one week,  repair or replace the 210 barrel tank, repair the connection on the poly tank, and rebuild the tank battery dike with good clay soil to the proper height to meet SPCC requirements.

| I was on location to follow up on compliance notice #1402942437.  Scott Fleishman (Enervest) informed me that the required work was completed on 6/27/14.  My inspection today found that the pinholes in the 210 barrel tank have been repaired and the dike around the tank battery has been rebuilt measuring 40'(L) x 12'(W) x 3'(H) and exceeds SPCC requirements for containing the 260 barrel capacity of the battery.  The gasket on the flow line to the 50 barrel poly has also been repaired.  The dike is currently holding water within the containment and there is no observable sheen on the water.  A small dike has also been constructed around the wellhead at the pumping unit.  The compliance issues at this well and tank battery have been resolved.  No violations.

| I received a phone call from Wendy Booth from our Uniontown Office that she received a call from Dan Flick stating that the Well Tender for Enervest was releasing fluid from the tank battery which was holding water inside the diked area and it was running on to his property. He stated that the lease road needs repaired. The lease road is passable for a water trucks and other well maintenance equipment.  Upon my inspection I could see where there was a trail of water that ran down the lease road but it did not appear to get on his property. When I tested the water at the tank battery is was very low ( fresh water ) Quan- tab showed very low. As for the lease road it does not appear to be on his property. I spoke with Joe Teppart Field Supervisor for Enervest about the lease road and also to stop draining the tank battery onto the lease road. Joe also stated that the lease road is not on his property. I tried to contact Mr. Flick but he has not returned my call. Complaint closed. Complaint number 4795.

The first one, I had Enervest fix last year and is beginning to leak again. The second, is on a welded seam below the first one and shows staining on the tank down to the ground. An absorbent pad was on the ground at the tank base to soak up the leaking product. Also, the connection to the poly tank is damaged and the gasket between the tank and the flange is broken in half and not in place. The landowner showed me the drainage path from the tank battery area towards his house to the east. An area of scattered, standing water, roughly 40 long through his yard had an oily sheen on top of it. I confirmed it was oil with an absorbent pad and with my fingers. Recent, heavy rain has mobilized residual oil inside the tank battery. The inabiblity of the dike to contain spills and releases inside the battery is apparent based on the presence of sheen 150 down slope of the tanks and the lack of any standing water within the battery. The height of the dike walls does not meet SPCC requirements and the southeast corner contains soil cracks that will allow the passage of liquids. Enervest Operating LLC will, within one week, repair or replace the 210 barrel tank, repair the connection on the poly tank, and rebuild the tank battery dike with good clay soil to the proper height to meet SPCC requirements.
1406809820 2014-07-30 Production Wells No Violations I was on location to follow up on compliance notice #1402942437. Scott Fleishman (Enervest) informed me that the required work was completed on 6/27/14. My inspection today found that the pinholes in the 210 barrel tank have been repaired and the dike around the tank battery has been rebuilt measuring 40(L) x 12(W) x 3(H) and exceeds SPCC requirements for containing the 260 barrel capacity of the battery. The gasket on the flow line to the 50 barrel poly has also been repaired. The dike is currently holding water within the containment and there is no observable sheen on the water. A small dike has also been constructed around the wellhead at the pumping unit. The compliance issues at this well and tank battery have been resolved. No violations.
1415893044 2014-09-10 Production Wells No Violations I received a phone call from Wendy Booth from our Uniontown Office that she received a call from Dan Flick stating that the Well Tender for Enervest was releasing fluid from the tank battery which was holding water inside the diked area and it was running on to his property. He stated that the lease road needs repaired. The lease road is passable for a water trucks and other well maintenance equipment. Upon my inspection I could see where there was a trail of water that ran down the lease road but it did not appear to get on his property. When I tested the water at the tank battery is was very low ( fresh water ) Quan- tab showed very low. As for the lease road it does not appear to be on his property. I spoke with Joe Teppart Field Supervisor for Enervest about the lease road and also to stop draining the tank battery onto the lease road. Joe also stated that the lease road is not on his property. I tried to contact Mr. Flick but he has not returned my call. Complaint closed. Complaint number 4795.
1632786698 2013-05-07 Production Wells No Violations I was on location to follow up on complaint ID#4291. The pin hole leak had been fixed and the tank and ground had been cleaned up. This complaint is closed. No violations.
