Present for the meeting were myself; Adam Blint, JKLM Land Manager; John Watts, J.L. Watts excavating; Jim Fowler and Al Pence, New Growth Resources. The weather was 65° partly sunny.

The Headwaters 145 Pad is an unconventional well site that will be constructed southeast of the existing Headwaters 143 well site. The permit also encompasses a natural gas gathering line that will connect the HW 145 to the HW 143 well site. As of today’s meeting, the 6 unconventional well permits submitted for the HW 145 pad do not show an issuance date in eFACTS. JKLM must have a valid well permit in hand before earth disturbance activities can commence. The start date for the project has not been scheduled at this time. 2547827 Primary Facility 2016-12-27 Preoperation Inspection No Violations Noted The Department of Environmental Protection conducted a pre-spud inspection at the JKLM Headwaters 145 Pad (ESG29-105-15-0007), located in Ulysses Township, Tioga Co on 12/27/2016 @ 1300 hours. The weather was 35°, sun/clouds, snow and ice on the ground melting rapidly. A site ID sign was observed at the entrance to the access road. A copy of the well permits, E&S plan and E&S inspection log was located in the mailbox onsite. The site was last inspected by company personnel on 12/14/2016 with the following comments “Access road and location being constructed. Signs in place, permits in box, MRP in box. Snow covered. No flowing or standing water. Access and location culverts being installed.”

The Headwaters 145 Pad is an unconventional well site that will be constructed southeast of the existing Headwaters 143 well site. The permit also encompasses a natural gas gathering line that will connect the HW 145 to the HW 143 well site. As of today’s inspection perimeter BMP’s consist of chipped material berms. The

2573309 Primary Facility 2017-03-20 Routine/Complete Inspection No Violations Noted The DEP inspected the JKLM Headwaters 145 well site on 3/20/2017 @ 1100 hours. Ron Burdick was the company rep onsite for JKLM. The HAD rig is currently onsite to drill the top hole portions of the 1HU-6HU wells. The rig has completed the drilling on the 1HU well and has skidded over onto the 2HU well. They were rigging up and preparing to drill on the 2HU at the time of this inspection. Mr. Burdick stated The HAD rig should be onsite until mid April. JKLM plans to bring the H&P 500 rig on location mid-Sept. to drill the horizontal portions of the wells. No violations are noted at this time. I departed the site at 1345 hours.

Containments were in good condition, and chemical storage areas were clean. Permit numbers were posted at site entrance.
2658617 Primary Facility 2017-11-07 Routine/Complete Inspection No Violations Noted A routine inspection of the of the JKLM Headwaters 145 well pad was conducted on 11/7/2017 @ 1330 hours. The weather was 32 degrees and snowing. JKLM is in the process of drilling the 3HU well. The rig was at a depth of 11,517' at the time of this inspection. The 4HU, 5HU and 6HU wells were drilled between 9/2/2017 10/19/2017. The 1HU and the 2HU have yet to be drilled. Mr. Joe Huston and Mr. Tyler Regan were present onsite during the inspection.

Fluid with an elevated conductivity was discovered in the NW corner of the rig containment below the rig mats. The field conductivity was measured between 3-4.5 mS/cm using an Oakton Multi-parameter PCSTesr 35. Mr. Huston and Mr. Regan were advised to remove the fluid with a vac truck as soon as possible. No elevated readings were detected outside of containment. The source of the fluid was not determined. No other accumulated fluid was found within the perimeter of the rig containment.

Additional E&S control along the access road may be needed to prevent sedime

2667407 Primary Facility 2017-12-05 Routine/Complete Inspection No Violations Noted Weather is 38 Deg. F., rainy and cloudy at time of inspection. Routine inspection of JKLM LLC, Headwaters 145 Pad on December, 5 2017.

There are total of 6 wells on location. Currently advancing 5-1/2" production casing on the 2HU well. The 1HU has not been drilled to completion as of this inspection. Drilled wells on location: 6HU drilling started on 07/25/17 and finished on 09/01/17; 5HU drilling started on 09/02/17 and finished on 09/29/17; 4HU drilling started on 09/30/17 and finished on 10/19/17; 3HU drilling started on 10/20/17 and finished on 11/16/17. We physically inspected the pad and the rig. The pad was looking neat and well organized. At the time of this inspection no date has been set for the drilling process to be resumed for the 1HU.

2704726 Primary Facility 2018-03-13 Routine/Complete Inspection No Violations Noted At the time of this inspection Scott West completions consultant was at this inspection. The 1HU,2HU,4HU and the 6HU were completed at this time.
2714782 Primary Facility 2018-04-04 Routine/Complete Inspection No Violations Noted I arrived onsite with Elizabeth Niles, WQS and met Scott West of JKLM. A coil tubing rig was on location; Scott informed us that they had finished drilling out plugs on the 6H and shut it in, and had just moved over to the 5H to begin drilling plugs on that well. Recent heavy rain had accumulated in containments and on the pad surface. Although the water in containment showed varied conductance (.60-11.1 mS/cm), puddles on the pad were consistently in the 2.2-3.1 mS/cm range. We noted a containment near the AST that had 20-40 gallons on fluid with elevated conductance (10.5 mS/cm); although water on the pad in proximity shower low conductance (.5-1.1 mS/cm), we did note that the containment had been constructed without a corner berm, and the liner had sagged into the corner. This created a potential path for this water to leave containment; I strongly recommend that JKLM utilize these corner berms, as it was exactly this type of berm construction that caused a release on the Reese Hollow 115 pad recentl
2715674 Primary Facility 2018-04-03 Routine/Complete Inspection No Violations Noted At the time of this inspection the hydraulic fracturing on this well was complete . A coil tubing company was on location setting up to clean out the wells after the plugs were drilled out.
2720016 Primary Facility 2018-04-18 Routine/Complete Inspection No Violations Noted At the time of this inspection the hydraulic fracturing and the coil tubing has been completed. The well was being set up for production.
2731590 Primary Facility 2018-05-17 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the JKLM Headwaters 145 well pad located in Ulysses Twp., Potter Co. on 5/17/18 @1100 hours. The weather was 72 degrees and sunny.

Due to recent rain events the pad surface was muddy with numerous puddles. Field Conductivity on the pad surface was screened and found to vary between 400 µS/cm – 4 mS/cm. The puddles exhibiting elevated conductivity were mostly located on the NE quadrant of the pad (see attached pictures). It is recommended the puddles exhibiting elevated conductivity be removed with a vac truck. The puddles around the AST were with in normal range.

Activity onsite consisted of installation of separators, production tanks and well tags. The site was last inspected by company personnel on 5/10/2018 with no issues noted. Containment around the well heads appeared intact and in good shape. Waste Management containers were located in the eastern corner of the pad and were tarped. Sediment traps appeared dry and not discharging.

2748765 Primary Facility 2018-07-02 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. Combustable gas was detected on the 9 5/8" x 13 3/8" annulus at this inspection. After a conversation with Jake Long (Engineer for JKLM) he informed me that this was not uncommon directly after flow back( a process of cleaning out the wells of hydraulic fracturing fluids). The cleaning out of the wells is under high pressure and temperatures during this process. On going monitoring will be conducted by JKLM until the wells have stabilized after the flow back process is completed and for an unspecified time there after.
2803610 Primary Facility 2018-11-15 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. There are four (4) annulus vents openly venting on this well. Annulus vent intermediate two (2) x intermediate one (1) (13 3/8" x 9 5/8") was venting 100% LEL (lower explosion limit) and 2% METHANE). Although the Intermediate 2 casing string was not cemented to surface under an approved alternate method, combustible gas detected at this annulus indicates potential defective cement. JKLM should monitor combustible gas levels at this annulus on a monthly basis and report any significant changes to the Department as soon as possible. No other annulus vents were noted venting at this inspection. No violations noted at the time of this inspection.
2838357 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. A gauge on the production/ flow line indicated a psi (pounds per square inch) of 1500. The 9" x 5" (intermediate x production) casing string is connected by a flexible approximate 2" vent line that is connected to the same size annulus on the other 5 wells on this well pad on all 6 vent into a poly vent tank at the end of the row of wells. No violations were noted at this inspection.
2840268 Primary Facility 2019-02-14 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the JKLM Headwaters 145 well pad located in Ulysses Twp, Potter Co. on /2/14/2019 @1145 hours. The weather was mostly sunny, 29 degrees, winds SSW @10mph, humidity 48%. Onsite activity consists of 6 wells that appear to be in production. An Above-ground Storage Tank (AST) was observed ~60% full of impaired fluid.

Poly tanks onsite, in containment, do not appear to be in use and are not labeled.

No violations are noted at this time. I departed the site 1230 hours.
