2441220 Primary Facility 2016-01-15 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:15 a.m., the cellar was full of ice; the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open with 0psi on it and the valve going to the tank was closed. There was 672psi on the production casing and 959psi on the production line. I took a walk around the site and everything looked OK.

I left the site at 11:30 a.m.

2740455 Primary Facility 2018-06-07 Routine/Complete Inspection No Violations Noted I inspected the Richard well site and performed a RTNC inspection. The well on location is the 115-20795. It was properly labeled.

The access road and pad surface were both solid and stable. The fill slope was very well vegetated. The cut slope was well vegetated except in an area which was quite rocky. There were no erosional issues noted.

There was a spoil pile on site but its not clear what purpose it served.

There were no violations.
