Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2818485 Primary Facility 2018-12-20 Routine/Complete Inspection No Violations Noted This location can be accessed from Old State Rd. in Mead Township, Warren County, Pa.

The ground is currently frozen with some thawing in certain areas. The portion of the accessing road that would have been most recently developed has become significantly rutted. It appears as though most of the stone used as road base was commercial and of a fairly small gage. It is advised that larger stone be added to the road to create a stronger driving surface. A spud was entered for today, 12/20/18, for this well. There is no one currently on location. The rig and pipe boat have been putt in place. No drilling, however, has occurred. The spud date will need to be updated, by the operator, 24 hours prior to when well drilling will actually take place. The drilling permit is clearly posted to a tree on the location.

2821129 Primary Facility 2019-01-02 Drilling/Alteration No Violations Noted No one on location at this time. Surface casing cement notification submitted for today, 01/02/2019 at approximately 13:00. Surface casing is suspended in well bore from drilling rig rotary head. Cementing wellhead is attached to surface casing with valve closed. Drill cuttings are contained in a temporary drill pit.
2842355 Primary Facility 2019-02-20 Routine/Complete Inspection No Violations Noted This location can be accessed from Old State Rd. in Mead Township, Warren County, Pa.

The location and accessing road are currently frozen and snow covered. The well is being prepared to frac. A pit has been dug on the cut bank side of the location. The pit and the entire area underneath the rig floor appear to be covered with plastic liner material. The drilling permit is clearly posted to a tree near the location.
