Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1907827 Primary Facility 2010-07-21 Drilling/Alteration No Violations Noted Northeast 10 - Td@ 5842, returns on cement, drilling ahead, site and location good,
2417831 Primary Facility 2015-10-22 Site Restoration No Violations Noted On 10/22/15, I performed a NOT inspection at the G Johnson site in response to a termination request from the operator and made the following observations:
2588041 Primary Facility 2017-02-23 Complaint Inspection No Violations Noted On 2/23/17, a complaint inspection was conducted in association with the Johnson Gerald Unit well site in response to CTS ID 324963; the following was observed:

•The complainant reported noticing a sheen on standing water in a roadside ditch roughly ½ mile from the well site, they suspect that their cat became sick after drinking from the ditch. The complainant provided photos of the sheen on the water but explained that it was not particularly visible to the naked eye. Subsequent photos taken from the complainants camera, the Department camera, and the inspector’s personal cell phone camera indicate the sheen is likely an artifact of the complainants camera as it did not appear in pictures taken simultaneously with the other two cameras. •Samples were taken from the ditch for SAC 0946 and bacteriological analysis. Sample ID 3401-036. •A large disturbed garden plot was noted 10-15 up gradient from the roadside ditch, the water in the ditch was sourced to a partially buried metal pipe. •The complainant als

2648797 Primary Facility 2017-04-12 Administrative/File Review No Violations Noted On 4/12/17 an administrative inspection was conducted in association with the Johnson Gerald Unit well site in response to CTS ID 324963; the following was observed:

>>Analysis of sample results for sample ID 3401-036 collected on 2/23/17 found no evidence of oil and gas impact; water parameters were consistent with sediment laden surface waters (sample collected from roadside ditch).
