0211530981 2016-12-08 Drill / Deepen / Reopen No Violations On the date of my inspection I found a well maintained access road with a locked gate. The pad is surrounded by an earthen dike with healthy vegetation and a fence with a locked gate. The pad has 2 unidentified newly drilled wells. Five frac tanks, 2 light generator units, and some miscellaneous containment pieces are currently on the pad. The wellhead is equipped with a partial christmas tree valve and is surrounded by a metal fence.

1289430789 2016-06-25 Casing String Witnessed No Violations I am on site today to witness the cementing of the conductor casing. Express Energy Services used an auger rig to drill a 24 inch diameter hole to a depth of 122 feet then ran 120 feet of 20 inch diameter conductor casing. Express Energy then pumped 230 sacks of Class A 15.6 pound per sack, 1.18 percent yield cement down hole and displaced with 39bbl of fresh water. During the displacement I did not witness cement returns until approximately 25bbl of displacement. I witnessed approximately 8bbl of good cement returns to surface. After Express was completed with the job, I witness the cement fell downhole approximately 17 feet. I stayed on site for 2 hours and discovered cement was staying at the consistent 17 foot down. Company man David Newman stated he would grout it back to surface later that day. I will return at a later date to check on the grout job performed.
1456868507 2016-02-23 Surface Facility Construction No Violations On my inspection today, I accompanied ODNR Engineers Paul Logan and Jerry Smith to check the status of well pad construction. The well pad is approximately 98 percent complete at final grade and still needs final rolling and gravel. Well pad slopes have been recently hydro-seeded. All erosion and sedimentation controls are in place. Signage has been placed at the access road entry point
1471612045 2016-08-19 Administrative Inspection No Violations Today I was onsite to follow up on a call from Paul Logan about possible leakage from the pad outfalls. I spoke with Chesapeake company man Chris about the issue. He indicated that to his knowledge the valves had not been opened recently. Upon inspection I found both outfalls to have a very slow leak. Chris indicated that he would have the valves checked.
1483626445 2017-01-05 Completion Testing No Violations On the date of my inspection I found FTS International rigged up onsite. They are onsite to hydraulically fracture both wells on the pad. Operations began on 1/4/17. All equipment is within proper containment.
1522072596 2018-03-26 Preliminary Restoration No Violations On the day of my inspection I found this pad containing two properly labeled wells. Surface and intermediate annular pressures read 2 and 458 psi, respectively. Production equipment includes two sand seperators, two GPUs, and two 400 barrel steel tanks, within their own steel containment. Production records are up to date. Pad is stable and vegetated. Site meets requirements on ORC 1509.072A for preliminary restoration
1762856031 2016-06-27 Casing String Witnessed No Violations On the day of my inspection following up on a conductor cement job I witnessed, I discovered Express Energy Services has grouted the backside of the conductor back to surface and left approximately 1.5 feet of cement in the tin horn.
