0291839394 2015-03-16 Production Wells No Violations I was called at 12:17 PM on 3/16/2015 by Joe Teppert of Enervest reporting that they had spill at the Smith #1 (34-029-20862-000) on 12th street in Knox Township, Columbiana County. He said an elbow on the separator dump line had blown and oil leaked down-
1041180664 2003-08-06 Production Wells No Violations The tank battery consists of 1-210 bbl storage tank, 1-concrete buried salt water tank, dike and a separator outside the dike.
1137527605 2006-01-13 Production Wells No Violations The tank battery consists of 1-210 bbl storage tank, 1-buried concrete salt water tank and a dike.
1511669381 2015-03-17 Production Wells No Violations I was onsite to conduct a follow-up status check to the oil spill on 3/16/2015. Remediation is going well. The pump truck was onsite in the morning skimming oil off the reservoirs created by the siphon dams. A sheen of oil could still be seen in the ditch and siphon dam reservoirs. Siphon dams installed on 3/16/2015 are working properly.
1794220097 2018-05-10 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0862. RBDMS indicated reported production was current. Access to the well and tank battery remained stable and drivable. The 8.625 well head was below grade. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. Staining consistent with a legacy operation was present. No live or mobile leaks were observed. A domestic supply connection was present. Production equipment consisted of one (1) horizontal separator, one (1) steel production tank and one (1) concrete brine vault. The separator remained in good repair and contained a current 31 day chart. A stable earthen dike that was free of fluid was present around the tank and vault. The steel production tank remained in good repair and contained correct identification. The tank was formerly a 210 bbl unit that was modified to a smaller volume. Valves were closed and plugged. The brine vault remained in good repair above grade and was holding fluid with freeboard. No violations were observed during this inspection.
