1157677801 | 2017-08-15 | Production Wells | No Violations | I did a status check inspection on this well. The well is equipped with a pumpjack with an electric motor. The tank battery consists of a steel production tank and an inground concrete brine vault. The well is legibly identified. There are no compliance issues noted at this time. |
1335879914 | 2012-04-30 | Production Wells | No Violations | At 10:32 AM I received a phone call from Inspector Brett Farnsworth (330-284-1409) concerning a lightning strike on an oil storage tank just East of Malvern, in Stark County. I started heading to the location and while in route Tim McConaha (Enervest Field Operations Manager) (330-903-4782) called me at 11:06 AM to advise me he was headed to the location. I arrived on site at 11:37AM and found the fire was out and the clean up had been initiated. The oil, approximately 2 barrels inside the dike and 1/2 barrel outside of the dike, was contained using sumps, absorbent pads and booms. The tank battery consist of one 210 barrel tank and one in-ground concrete brine tank. The top on the tank was blown off and split into 2 pieces, which landed Northeast of the tank battery with one piece 86 yards away and the other 70 yards away. The power to the pumping unit has been shut-off and the well shut in. Wade Balser, of the Ohio EPA, arrived at 12:20 PM. The intial call was received by the Waynesburg Fire Department at 10:20am and they were on location when I arrived. Also on location were J. Hawk Water Service with a vac truck for clean up, and Don Himes (Enervest Pumper). |
1337259524 | 2012-05-17 | Production Wells | No Violations | This is my follow up inspection for a lightning strike on the tank battery. My inspection found the tank battery has been replaced with a 140 barrel tank and the clean up is complete outside and inside of the SPCC dike. There are no violations at this time. |
