1086294385 2004-05-26 Production Wells No Violations CASE 04-022 On 5/22 at 01:52, during a severe thunderstorm, the Kent Fire Department was dispatched to a fire at 2262 Lake Rockwell Road. The fire was extinguished and the last unit left by 02:40. Kent Fire notified OEPA and Akron Watershed of the incident. The Uniontown office received a call from Jim Erwin (330-789-9281) with OEPA on 5/26 reporting a lightning strike and possible oil spill at a tank battery operated by Beck Energy. The well involved is the Granc #1, P/N 133-2699. The tank battery consisted of one 210 bbl tank, two underground concrete vaults and one horizontal separator. The 210 bbl tank was struck by lightning, which vaporized the contents and sent the tank 435 feet to the east northeast. There was no evidence of a spill where the tank landed nor was there any affectment between the tank battery and the landing zone. The resultant fire consumed a majority of the liquids initially released at the tank battery. Becks initial response on 5/22 was to shut the well in and start containing any residual liquids with sawdust. The damaged equipment and affected soil will be removed and disposed.

1122986351 2005-07-21 Production Wells Violations Noted CASE 05-019 The Uniontown office received a call from Tim Harrod (330-815-1474) at 2262 Lake Rockwell Road reporting a gas leak at the wellhead that has gotten progressively worse. The leak is at the tubing head and around the tubing collar. I contacted Beck and told them to repair the leak.
